5 Accident and Health Term Life Whole Life Annuity Medicare Supplement

The policy pays a one-time death benefit when death occurs from an accident. The cost is only $10 a year for the entire family. ADP is designed for every prospect ages 18-68 and is a tremendous low-cost prospecting tool. It is particularly valuable for individuals unable to afford any other type of coverage. Accidents are among the leading causes of death for young and middle-age Americans. Should the unthinkable occur, a $3,000 Accidental Death Policy can be invaluable for someone with limited financial resources.

Record the names, Social Security numbers, and dates of birth for named insured, spouse, and children on the application. Coverage becomes effective when the named insured or spouse and the Agent sign the application.

There is no money collected at the time of application. Premium is payable at the end of the policy year. The policy can only be billed on annual mode.

Full coverage until the policy anniversary following the insured's 70th birthday.

There are no commissions, renewals, or bonuses payable for this policy.

ELIGIBILITY

A child is less than 21 years of age, unmarried, and lives with primary insured or is primarily dependent upon primary insured for support. Legally adopted children and stepchildren are eligible. There are no health questions on the application. The policy is not underwritten. Customers and their family members are not eligible to be covered under more than one ADP at a time. If ADP lapses, the customer is not eligible for a new policy for three years. BENEFIT OVERVIEW

Primary insured $3,000, spouse $3,000, each child $1,000. Limit one policy per family.

The insurance on any child covered under this policy terminates at the earlier of the termination date of the policy or the child's 25th birthday.
