Description

Informal and interactive overview of what a medical home is and the resources available to support physicians and the children and families they serve, especially those with special health care needs. Kate Orville, Co-Director of the Washington State Medical Home Leadership Network follows a general outline residents have found helpful and adapts it based on the individual experiences and interests of the residents and medical students in each session. Participants learn how to use the WA State Medical Home website as a resource for caring for their patients.

Educational Goal

• Acquire a clear understanding of the medical home model of health care, its benefits for providers and their patients, and why the medical home is especially important for children and youth with special health care needs and their families

• Understand your role in a medical home

• Become aware of key community resources and office tools available to support physicians as medical homes (or specialists co-managing care with a primary care provider)

Learning Objectives

Because of participating in the Medical Home discussion, trainees will be able to:

a. Patient Care

• identify at least two ways to quickly find contact information for resources for families in the community in which they live.

• describe at least 2 tools physicians can share with families to help organize their child’s health information & coordinate care among providers.

b. Systems-based Practice

• describe how three key community service providers – public health, Birth to Three early intervention services, and parent to parent support -- can help with no-cost care coordination and support for families who have children with special health care needs

• navigate the WA State Medical Home website as a tool to support medical homes.
