CONTINUATION PAGEA.1 Performance Work StatementPerformance Work Statement (PWS)Moving Services - Cheyenne VAMCThis is the guaranteed minimum amount for the life of this IDIQ contract.The minimum order for Moving Services is 100 hours during the first ordering period.The maximum aggregate ceiling value for this IDIQ contract is 3,000 hours over the five (5) year ordering period.The Government does not guarantee that it will place any order under this contract in excess of the guaranteed minimum award amount.Description of ServicesThis Performance Work Statement (PWS) defines the requirement to provide Moving Services. A moving contract is required to facilitate moves throughout the facility and its entities. Scope of WorkThe contractor shall provide all personnel, equipment, tools, materials, vehicles, supervision, certifications, and other services to provide moving services. The contractor shall perform to the standards in accordance with this SOW. Requirements:2.1. Moving staff and equipment will be scheduled when services are required, no less than 1 day a month. The moving contractor can anticipate approximately 4-8 hours of service work per month during the VA business hours of 8 a.m. and 4:30 p.m. 2.2 The COR shall provide the contractor with a list and location of items to be moved, a minimum of one hundred sixty-eight (168) hours in advance of every move. The list will include the location of identified items, the designated area in which items will be moved, and the placement of said items in the designated area. The Contractor will provide a plan of action to corroborate with COR instructions seventy-two (72) hours prior to all scheduled moves. 2.3 Contractor shall move all items (desks, file cabinets, chairs, etc.) as outlined in the list provided by the Contractor Officer Representative (COR).2.4 Contractor will determine the number of personnel and the type of equipment needed to handle all moving requirements, while ensuring proper safety. If a move detail has already been scheduled, the Cheyenne VAMC reserves the right to cancel the move request and will provide a forty-eight (48) hour notification of cancellation.2.5 The Cheyenne VAMC estimates (400) hours per ordering period of scheduled work. Pay rate will be hourly and the schedule of these man hours will be determined by the Cheyenne VAMC using the notification guidelines set forth above. Currently there are two anticipated move dates per month with additional moves added as needed.Specific TasksContractor is required to furnish the following items: 3.1 All tools needed to properly move a variety of items. These tools include, although are not limited to: pallet jacks, desk jacks, dolly, moving skates/traps, and a moving truck(s). 3.2 All tools to perform minor tear down and reconstruction of identified items to better facilitate the identified items to be moved.3.3 All personal protective equipment (PPE), i.e.: safety boots, gloves, etc. for movers are to be worn when moving identified items.3.4 All personnel shall have working knowledge of moving tools being utilized and knowledge of proper moving safety techniques.3.5 The Cheyenne VAMC will relocate and stage large items via forklift that cannot be moved by moving personnel. Any items that needs to be transported by vehicle will be loaded and unloaded by contractor. Contractor will provide the truck(s) for moving. The Cheyenne VAMC will provide a forklift and an operator for the forklift when applicable.Performance MonitoringThe COR or a POC (designee) will be present for each facility move. The COR/POC will ONLY observe the moves in the facility and at Cheyenne VAMC entities. Performance is measured by direct observation of movers. Any negative performance will be reported to the moving organizations management to be addressed and to allow for correction at the contractor’s discretion. Security RequirementsContractors may be required at times to enter sensitive areas in the process of moving furniture and other items. At no time will contractors have access to patient records, data or VA computer systems. All patient sensitive information is to be secured by staff prior to movers entering any area. Contractors will check-in with VA police to get visitor badges for each move ernment-Furnished Equipment (GFE)/Government-Furnished Information (GFI).There will be no Government-Furnished Equipment (GFE) or Government-Furnished Information (GFI) used by contractors.Inspection and Acceptance CriteriaThe COR is responsible for certifying all work done under the contract is performed on-time and in accordance with standards. The COR is also responsible to assure the inspection and acceptance of products provided incidental to services.Property Damage The contractor shall take all necessary precautions to prevent damage to any government property and will notify the Contracting Officer immediately if damages occur. The Contracting Officer will authorize the contractor to remedy the situation in one of the following ways: be assessed current replacement costs for damaged property, replace damaged property in a timely manner, or correct the damages with like materials at no additional cost.Price/Cost QuoteInterested vendors shall include the following information in their quotes:Due to concerns resulting from acts of nature, this PWS is amended as follows:All mileage rates are hereby removed, hourly rates are modified to cover the needs of the Cheyenne VAMC’s moving requirements contract.Labor Hourly Rate for internal moves within the Cheyenne VAMC when a truck is not required.Truck Hourly Rate for all moves between VA facilities when a truck is required. This rate shall include two employees, loading and unloading of trucks, fuel cost, and any other cost associated with moves between facilities. All additional employees required for moves between facilities will be paid the Labor Hour Rate. If multiple trucks are required, the truck rate employee requirements will be tiered with each additional truck.Example: Two trucks equals four employees, three trucks equals six employees, etc… All additional employees above the minimum requirement will be paid the Labor Hour Rate mentioned above.An hourly rate for the Employee(s) while loading and unloading at the Cheyenne VAMC and/or its entities and while moving items within any of the facilities listed below.An hourly rate for the Truck(s) while loading/unloading at the Cheyenne VAMC and/or its entities.A round trip mileage rate between the Cheyenne VAMC and its entities.
