Eligibility Criteria forHurlburt Field Chief’s Group 2016 Book grantScholarship applicants must meet the following criteria1. An enlisted member in the grades of E-1 through E-4. 2. Be enrolled/attending a college or university that does not provide a book grant to its students.3. Complete the book grant application. 4. Understand the policy letter for acceptance of the book grant.APPLICATION PROCEDURES1. Complete pages 4 and 5 of this application2. If emailing package use HCG Book Grant as the subject of the message and send the application pages as an attachment. Please ensure the two pages remain separated by a page break.SELECTION PROCEDURESJudging will be by an impartial committee of at least three members of the Chief’s Group working independently. No committee member will be related to any applicant. Judges will base their selections on the applications submitted. Applicant’s name and contact information will not appear on any portion of the application seen by the judges.The book grant recipient will be selected based on:Verification of eligibility.Application completed in full.Financial need.Judges’ overall evaluation of the application data.POLICY LETTERFORHURLBURT FIELD CHIEF’S GROUP BOOK GRANTS1. This book grant is based on scholastic and professional goals, financial need and extracurricular activities and interests.2. Four book grants of $250.00 will be awarded to recipients who meet the eligibility requirements.3. Official, written verification of the recipient’s registration/enrollment must be received by the Chief’s Group prior to distribution of funds. A copy of the approved TA form or confirmation from either the base education office or school will meet this requirement.4. The winner (s) will be notified prior to receiving the award. If the original winner declines acceptance of the book grant, a subsequent winner will be notified as soon as possible. Failure to respond to the Chief’s Group Scholarship Committee within 20 days of notification with acknowledgement of acceptance of the book grant constitutes a declination of the scholarship.5. Winners will receive a check for $250.00 to apply towards the purchase of textbooks required for college courses the students are taking. New or used books may be purchased from any source available. They do not have to be purchased from the college book store.6. Students will keep a log of text books purchased (log will be provided by the Chief’s Group). Once all funds have been expended, the form will be sent to the scholarship committee for filing.7. Any student planning delayed school enrollment must notify the Chief’s Group Scholarship Chairperson and the book grant money will be held in the student’s name for six months after notification.
