VTE ROP

• VTE is the collective term for deep vein thrombosis (DVT) and pulmonary embolism (PE).

• It is a serious and common complication for patients in hospital and/or undergoing surgery.

• Evidence shows that identifying patients at risk and providing evidence-based thromboprophylaxis interventions, can substantially reduce or prevent thromboembolism from occurring.

Tip:

When completing a risk assessment on a patient, consult the most up-to-date clinical order set to identify which thromboprophylaxis intervention to use.

Did you know:

• Health PEI has VTE Prophylaxis Guidelines which set out Inclusion and Exclusion criteria

• A VTE Prophylaxis order set and ORTHO VTE prophylaxis order set exist in the electronic system.

• VTE order sets can be ordered on their own. They are also a part of 53 other order sets. For example GENSURG Minor Surgery Post op, ICU Ventilation, PAL Palliative Care Admit and Sepsis Management

• “Safer Healthcare Now” Identifies the following as risk factos for VTE: major surgery, trauma or leg injury, active cancer, acute medical illness, immobilization, bed rest, stroke, previous history of VTE, increasing age, pregnancy, birth control pill, hormone replacement therapy, severe obestiy, thrombophilia and central venous catheter.

Required Organizational Practices (ROPs) are identified by Accreditation Canada as evidence-based practices, which reduce harm to patients. They are mandatory practices matched to specific Accreditation Canada Standards that organizations must have in place. There are 33 ROPs identified within Health PEI for the September 2017 survey visit.
