Important Information: please read before collecting your hair sample.

? 0.5 gram of hair is required for analysis (approximately 1 tablespoonful). ? Hair should be clean and dry. ? Hair can be taken from anywhere on the head; the back of the head is least

conspicuous. You can cut small amounts from different areas if preferred. ? The 3cm of hair nearest the scalp is best for analysis. For long lengths of hair,

cut off and discard anything other than the 3cm closest to the scalp. ? Use clean stainless steel scissors to cut your hair. ? Chemically treated hair can be contaminated, therefore we do not recommend

submitting coloured, permed, bleached or otherwise treated hair for analysis.

Collecting your hair sample

Complete all details on zip lock bag label

With clean, dry hands, cut hair from the back of the head as close to the scalp as possible, retaining the 3cm of hair nearest the scalp for analysis. After each cut, place hair in the labelled sample bag.

When you have collected approximately a tablespoonful, carefully seal the bag. It is better to have more hair than not enough.

Complete Analysis Request form (on reverse of this sheet).

Place zip lock bag inside the envelope along with completed Analysis Request and cheque/money order (if not credit card).
