This funding allowed me to travel to Paris, Grenoble, and Poland to speak with survivors and interview them. This honors thesis was submitted jointly to the Departments of History and French in 1993, and I graduated Magna Cum Laude, based on a combination of coursework and this thesis.2007-2010Global Women’s Health FellowshipGlobal Women’s Health Fellowship (Division of Women’s Health, BWH)PI/FellowThis fellowship funds gifted junior-level investigators pursuing research in global women’s health issues for travel to resource-limited settings as well as intensive training in research methodology at Harvard’s School of Public Health. This funded research focused on HPV vaccine adherence among adolescents in resource-limited settings. One manuscript has resulted from this work, and two more are under review.2008-2009Post-Doctoral FellowshipT32 post-doctoral Fellowship (National Institute of Allergy and Infectious Disease) / T32 AI007433-16PI ($42,000 per annum – up to 2 years)This award funded research focused on prevention of mother-to-child transmission of HIV and factors impacting HIV prevention and treatment. Two manuscripts resulted from this work.2009-2011Adherence to the Human Papillomavirus (HPV) Vaccine in a Resource-Limited SettingKL2 Medical Research Investigator Training (MeRIT) Program (Harvard Catalyst) / KL2RR025757PI ($90,000 per annum over 2 years)This early career development award provided me with 75% salary support to?develop a behavioral model of vaccine adherence in an adolescent population at risk for?HIV acquisition.?This mixed-methods research project, based in Soweto, South Africa, led to the creation of a conceptual framework and a psychometrically sound scale to measure factors impacting adherence,?with the ultimate aim of helping?guide the successful global expansion of the HIV vaccine upon its availability. This work was a continuation of the Global Women’s Health Fellowship, and led to two manuscripts.2010-2012Adherence to the HPV Vaccine in a Resource-Limited SettingHarvard Global Health Institute Travel Award (Harvard Initiative for Global Health)PI ($20,000)This award provided me with research and travel support to?develop a behavioral model of vaccine adherence in an adolescent population at risk for?HIV acquisition.?This award supplemented the KL2 Merit Award and provided additional support required for travel to South Africa.2010-2013Eleanor and Miles Shore 50th Anniversary AwardEleanor and Miles Shore 50th Anniversary Award (HMS)PI ($25,000)This award provided support to?launch my academic career at HMS. This award furthered my research project focused on HPV vaccine uptake. It is a mixed methods research project, based in Soweto, South Africa, that led to the creation of a conceptual framework and a psychometrically sound scale to measure factors impacting adherence,?with the ultimate aim of helping?guide the successful global expansion of the HIV vaccine upon its availability.2012-2013Harvard University Center for AIDS Research Scholar AwardHarvard University Center for AIDS Research Scholar Award (Harvard University)PI ($20,000)This award provides research support to supplement my ongoing work in Soweto, South Africa focusing on HIV treatment refusal. One manuscript resulted from this work.2013-2015Improving Adolescent HPV Vaccine Series Initiation and Completion in Pediatric Clinics (PI: Mark A. Schuster)R24 (National Institute of Minority Health and Health Disparities) / R24MD001648Scientific AdvisorMy role as a scientific advisor on this grant is to assist in the qualitative research to be performed that will guide development of an evidence-based clinic intervention to improve HPV vaccine initiation and completion rates among a racially/ethnically diverse low-income sample of adolescent boys and girls aged 11-17. Ultimately, this work will lead to a demonstration project to examine whether an evidence-based clinic intervention improves HPV vaccine initiation and completion rates overall and by gender, race/ethnicity, and insurance status.2015Systematic review on adolescent friendly HIV health servicesWorld Health Organization Contract(PI: Lisa Butler)My role as a scientific advisor on this grant is to assist Dr. Butler to review literature, summarize findings, and prepare a presentation for the World Health Organization Guidelines meeting. Current2012-2017HIV Treatment Refusal Among Adults Presenting for Testing in Soweto, South AfricaNIH K23 / 1K23MH097667PI ($921,846)My goal with this K23 award is to identify modifiable determinants of treatment-refusal, and design a targeted and informed intervention to counteract this phenomenon in Soweto, South Africa and prevent premature mortality in this vulnerable population. Using a mixed-methods approach, I will identify modifiable psycho-social determinants of antiretroviral therapy (ART)-refusal by performing in-depth qualitative interviews with 40 ART-eligible adults presenting for voluntary counseling and testing (VCT), and 10 healthcare providers, to inform category construction for a putative model based upon Anderson's Model of Healthcare Utilization. I will then examine correlates and outcomes of ART-refusal in 500 treatment-eligible adults, through survey implementation at the point of testing and for 6 months post-VCT. Finally, I will develop an intervention to promote HIV treatment acceptance among newly diagnosed HIV-infected individuals and pilot test it in a population of 100 treatment-eligible individuals.2015-2018 The Treatment Ambassador Program: Pilot testing a peer-driven intervention to increase treatment initiation among HIV-positive South AfricansNIH R34 / 1R34MH108393PI ($689,190)The research proposed here seeks to capitalize on our understanding of treatment refusal among people living with HIV in South Africa by designing a feasible and acceptable intervention to improve ART initiation. This multi-component intervention, titled the “Treatment Ambassador Program” (TAP), will target HIV+ people who do not initiate treatment within six months of learning they are eligible. It will be aimed at addressing barriers to ART initiation identified through our prior qualitative research, as framed through the Theory of Triadic Influence. It will be tested through a pilot randomized controlled trial to determine the acceptability, feasibility and preliminary impact of the intervention on ART refusal.2015-2017Unyango Iqabane – Developing a peer driven model of treatment initiation among people living with HIV in South AfricaHarvard Global Health Institute / Burke Global Health FellowshipPI ($74,654)The project proposed here is a one-year pilot study of a community-based combination treatment intervention that addresses both psycho-educational and structural barriers to treatment initiation and viral suppression for HIV-positive young South Africans. 2017-2018Harvard University Center for AIDS Research: Standing Tall with South Africa Harvard University Center for AIDS Research Scholar AwardPI ($69,824)The project proposed here lays the formative work for a socio-behavioral randomized controlled trial to test a peer-based intervention to assist individuals living with HIV in South Africa initiate treatment.Projects Submitted for FundingPending (2017-2020)Standing Tall - A Pilot Randomized Controlled Trial of a Community-Based Intervention to Improve Health Outcomes for Newly Diagnosed HIV-Positive Young Adults in South AfricaNIH R34 / NIMHPI (Direct costs requested: $511,657)The proposed study is a pilot randomized controlled trial of “Standing Tall,” a combination intervention strategy, informed by Social Action Theory, focused on ensuring that young people living newly diagnosed with HIV in South Africa (ages 18-24) initiate treatment in a timely manner, and achieve durable viral load suppression, retention in care, and sexual risk-reduction behavior. Standing Tall combines psycho-educational support, provided by lay counselors, peer support, and treatment provision in the context of a youth-focused Treatment Club across five high HIV-prevalence urban communities in the Western Cape Province.Report of Local Teaching and TrainingTeaching of Students in CoursesHarvard Medical School2010-2012Faculty Evaluator in Patient Doctor IIHMS StudentsBWH4-5 hours per month for 2 months per year2013-2015Faculty Tutor in Introduction to Social Medicine and Global Health - SM 750HMS Students2-3 sessions in Fall semester 2014-Faculty Mentor in the Scholars in Medicine ProgramHMS StudentsHMS5 hours per month for 9 months per year2015-Faculty Facilitator in the Young Leader Global Health WorkshopHarvard University StudentsHU1 week-long workshopFormal Teaching of Residents, Clinical Fellows and Research Fellows (post-docs)Harvard2009-2010Instructor - Humanistic Curriculum 15 Medical InternsHMS, BWH3-4 hours per month2012-Faculty of Epidemiology 208, Program in Clinical Effectiveness5 Research FellowsHarvard T.H. As a physician-scientist, board-licensed in Internal Medicine and Infectious Diseases, I have focused my research over the past decade on the social determinants of health-seeking behavior among HIV-infected individuals in sub-Saharan Africa, and improving care for the most underserved. My work has been predominantly centered in South Africa, where our team identified the phenomenon of “treatment refusal” among individuals presenting for HIV testing. I am currently funded through a National Institute of Mental Health (NIMH) K23 Career Development Award (K23MH097667) to determine factors driving HIV treatment refusal among individuals presenting for voluntary counseling and testing in South Africa. In addition, I am the recipient of an NIMH R34 (R34MH108393) to develop and test a peer-based intervention, called “The Treatment Ambassador Program,” which focuses on bringing individuals living with HIV into care sooner, and a Burke Fellow through the Harvard Global Health Institute. I have written several perspectives for the New England Journal of Medicine (NEJM), where I served as an Editorial Fellow and am currently on the advisory committee. To date, I have completed or have in preparation over forty original research manuscripts, perspectives, and editorials, including publications in NEJM, Annals of Internal Medicine, Lancet HIV, and AIDS. Our research has also been featured in UNAIDS publications, and throughout the lay press including Scientific American and Reuters. I have presented our research at international conferences, including the International AIDS Society Conference (Durban, South Africa, 2016) and the International Conference on HIV Treatment and Prevention Adherence, as well as national conferences and NIH-sponsored symposiums. I was selected to join the Early Career Reviewer (ECR) program at the Center for Scientific Review (CSR), NIH, and to participate in a highly competitive program through the Office of Behavioral and Social Sciences Research at the NIH on Randomized Behavioral Clinical Trials. My research is informed by my training as a physician caring for individuals in underserved communities in Boston, with a focus on community-based strategies to improve outcomes in care. Two months of each year are devoted to direct patient care. During this time, I act as a clinical attending on the inpatient medicine service at BWH and Brigham and Women’s Faulkner Hospital. I also supervise and teach medical students from HMS, as well as house staff from BWH in bi-weekly hour long didactic sessions as well as provide direct guidance on oral presentation skills. In addition to my research and clinical work, I hold leadership roles at Harvard-wide programs, and provide mentorship for students, trainees, and junior faculty. I am the organizing founder of the Biannual Global Women’s Health Research Retreat, which I have run since its inception in 2011. This research retreat is an initiative designed to bring together junior investigators across all Harvard-affiliated hospitals and provide them with direct access to senior-level investigators involved in similar research. Through this network, I have fostered the development of over 30 junior investigators. In recognition of this effort, my Division Chief nominated me for a Pillar Award. Additionally, I directly mentor medical students (through the Scholars in Medicine program), residents, and fellows both through my research and my clinical work in the U.S. and South Africa. I have served as a Faculty of Epidemiology 208 through Harvard T.H. Chan School of Public Health Program in Clinical Effectiveness, and I also play an active leadership role as the former Faculty-Chair (2015-2016) of the Joint Committee on the Status of Women, through HMS. As a member of the Division of Medical Communications, I have been a member of the Perspectives Working Group at NEJM, and continue to publish articles in the lay press, including The New York Times, The Huffington Post, and Slate, and have provided commentary for National Public Radio on topics related to HIV and global health. ................
