Using your Security Health Plan Directory

Directory sections

Security Health Plan works with doctors and other health care professionals, clinics and hospitals to help you and your family get the health care you need. This directory is sectioned by:

? Primary care providers (PCPs) ? Specialty care providers ? Behavioral health providers ? Ambulatory surgery centers ? Hospitals/privileges ? Urgent care centers ? Other plan providers (durable medical

equipment, orthotics/prosthetics, home health agencies and skilled nursing facilities) ? Retail clinics Each section is divided by city so that it is easy for you to locate participating providers near your home or work. In addition, we have provided an index of primary and specialty care providers by specialty in the back of this directory.

Choosing a primary care provider

This directory will give you information about choosing a primary care provider. Primary care providers are doctors, physician assistants or nurse practitioners. Your primary care provider should serve as your first contact for medical care and should be someone you see on a regular basis. Your primary care provider will coordinate your health care, including referrals to doctors within our network. Primary care providers are medical experts in:

? Adolescent medicine ? ages 13 to 20 ? Family practice/family medicine ? all ages ? Internal medicine ? usually age 18 and older ? General practice ? all ages ? Pediatrics ? usually birth to age 18

Although every effort has been made to provide the most accurate and up-to-date information, participating physicians are subject to change without notice. We recommend that you contact the office of your selected physician to be sure that physician is participating in the Security Health Plan network specific to your plan, and is accepting new patients.
