Get started with your OTC Flexible Benefit

Every three months, we'll add a $51 allowance to your OTC Member Rewards Card to spend on items in this catalog. The allowance will be added to your account on each of these dates:

? January 1 ? April 1 ? July 1 ? October 1 Be sure to use all of your allowance every three months. Any remaining OTC Flexible Benefit dollars will not carry over into the next three-month period.

Get Amerigroup Member Rewards for doing healthy activities

Complete a healthy activity from the chart below and we'll add Amerigroup Member Rewards dollars to your OTC Member Rewards Card. Here's how to complete an activity:

1. Visit your primary care provider and complete any of the healthy activities. Call Member Services toll-free at 1-855-878-1784 (TTY: 711), Monday through Friday, from 8 a.m. to 8 p.m. local time to see if you qualify for certain rewards.

2. Your doctor will send us a bill for the healthy activity.

3. Once we get the bill, we'll add the Amerigroup Member Rewards dollars to your OTC Member Rewards Card.

4. Use your rewards to purchase items in this catalog.

Healthy Activity

Attend the in-person or by-phone Inter-Disciplinary Team (IDT) meeting with your service coordinator and primary care provider (optional) to talk about your care plan.

Visit your primary care provider for your annual checkup.

Get your annual flu shot.

Call your service coordinator within three days of the day you were released from a hospital, skilled nursing facility, or nursing home to complete the post-discharge process. To reach your service coordinator, call 1-855-878-1784 (TTY: 711), Monday through Friday from 8 a.m. to 8 p.m. local time. The call is free.

Visit your primary care provider within 14 days from the day you were released from a hospital, skilled nursing facility, or nursing home.

Get a retinopathy eye exam, kidney exam, and HbA1c blood test; all three exams must be completed to qualify.

Get a blood pressure reading under 140/90 and fax it to Amerigroup Member Rewards at 1-844-206-3448.

Get a breast cancer screening.

Get a colorectal cancer screening.

Q: What's the difference between my OTC Flexible Benefit and Amerigroup Member Rewards? A: Your OTC Flexible Benefit gives you up to $51 to order OTC items every three months. You don't have to

do any special activity to get this benefit. With Amerigroup Member Rewards, you have to do a healthy activity, like your annual checkup with your primary care provider, to earn rewards. Review the chart on page 3 for the complete list of healthy activities. Q: How often can I place an order? A: We suggest you place one order every month. If the total amount of your order is more than your unused OTC Flexible Benefit allowance plus any earned Amerigroup Member Rewards dollars, you will need to pay the extra amount with another form of payment. Or you can ask the cashier to remove items that exceed your available balance. To find out how much you have in your account, call 1-866-413-2582 (TTY: 711) 24 hours a day, 7 days a week.

Q: How long does it take for items to get sent to me after I order them? A: Your order will be processed within two to five business days, and it can take up to 10 business days for you to

get your items. Your items will be sent by U.S. Postal Service mail, and each order will be tracked for delivery.

Q: What happens if I don't use all of my allowance every three months as part of my OTC Flexible Benefit? A: Amerigroup STAR+PLUS MMP puts money into your OTC Flexible Benefit account when you first enroll and again

every three months (each quarter). Unused funds do not carry over from one three-month allowance period to the next. Any unused amounts expire at the end of each quarter and on December 31 of each year.

Answers to your questions about Amerigroup Member Rewards

Q: Once I complete an Amerigroup Member Rewards activity, how do I get my reward dollars? How long does it take for them to be available?

A: That depends on how quickly your doctor sends a bill to Amerigroup STAR+PLUS MMP. Once we process the bill, we'll add the reward to your card on the first business day of the following month. We'll also send you a letter notifying you of the reward. Letters are sent monthly.

Q: Do my Amerigroup Member Rewards dollars ever expire? A: Yes, they expire at the end of every year (December 31). Members should complete any healthy activities by

November 1, 2020 to allow for processing and using the reward dollars by December 31, 2020.

Q: How do I know how much money I have left on my OTC Member Rewards Card? A: Your OTC Member Rewards Card will include funds from your OTC Flexible Benefit and any earned Amerigroup
