HealthFlex: A Comprehensive Approach to Healthy Living

HealthFlex is much more than a health plan. It offers a comprehensive approach to a healthier lifestyle--with features that support disease prevention, physical vitality and emotional well-being.

The HealthFlex focus on prevention helps clergy, lay employees and their families prevent disease and improve their overall wellness. In addition to providing coverage for medical care, behavioral health, prescription medications, vision care* and dental care**, HealthFlex provides a broad menu of wellness benefits, including: ? health screenings, ? coaching services that address health risks and chronic conditions, ? a physical activity program that delivers financial rewards, ? access to stress management services, ? access to financial and legal consultation, ? referrals for elder and child care services, and ? health information tools and resources.

Many of these services are available to HealthFlex participants and their families at no out-of-pocket cost.

* Basic vision coverage is provided to all HealthFlex participants. Some plan sponsors elect to provide more comprehensive vision benefits.

This brochure presents an overview of the suite of wellness programs and services available to you as a HealthFlex participant or covered spouse--as well as details on how to access these services.

If you think health care benefits matter only when people get sick--think again. Preventing illness and keeping people healthy are primary goals of HealthFlex. Don't wait to take advantage of HealthFlex's depth of resources aimed at preventing illness and chronic health problems.

3 Step 1: Blueprint for Wellness--The Quest Diagnostics Blueprint for Wellness? screening is the first HealthFlex step toward a healthier you. This screening involves a single blood draw, followed by lab analysis for more than 20 health indicators, including blood glucose, cholesterol, hemoglobin, PSA (men only) and much more. Test results provide valuable information about heart disease risk, diabetes risk (or diabetes management for those who have diabetes), kidney health and liver health. The Blueprint for Wellness screening may be more comprehensive than your doctor's standard blood tests--and is available to you at no out-of-pocket cost. Please share your Blueprint for Wellness results with your doctor.

You may be able to take the Blueprint for Wellness screening at an on-site event if offered by your plan sponsor--or you can go to a local Quest Diagnostics facility.

To register for a Blueprint for Wellness screening, call 866.908.9440 (employer/program: "United Methodist Church" or "HealthFlex"). You also can register through the HealthFlex/WebMD website. Go to , click on "HealthFlex/WebMD" and enter your username and password (or choose "New Users--Register Here" if this is your first time on the website). Then select "Blueprint for Wellness (Quest)" under "HealthFlex Vendor Links." (Registration key for 2011: UMC2011. Have your medical insurance card handy for online registration.)

3 Step 2: HealthQuotient--The WebMD HealthQuotient (HQ) online health risk assessment is your next step toward preventing disease and managing health risks. This online tool is a fast and easy way to evaluate how personal health factors, lifestyle habits and family medical history may raise your risk for serious conditions like heart disease or diabetes. The HQ does not diagnose conditions--but it does provide information about how habits or family history may increase your likelihood of developing certain health conditions. It may also help you prioritize which risk factors deserve your attention first.

The HQ is easy and confidential--simply enter your information through a secure website. If you took the Blueprint for Wellness screening, some of your health data will be conveniently and securely uploaded into your HQ.

Your HQ answers are summarized to generate a report that includes an overall HQ score as well as an assessment of your potential risk for conditions

such as diabetes, heart disease, certain cancers or depression. HQ takes things a step further, with practical suggestions for improving your scores-- and your wellness--in target areas.

Your Privacy Is Valued We are committed to keeping your personal health information private. Your church, annual conference, employer, HealthFlex, the General Board of Pension and Health Benefits (General Board) and your insurance company cannot access your lab results, answers to health questionnaires or other personal information. HealthFlex partner vendors--such as WebMD and United Behavioral Health--are bound by federal HIPAA privacy laws.

3 Step 2: HealthFlex Benefit: Wellness Exam--All HealthFlex medical plans include a provision for an annual wellness exam that includes an office visit plus all coordinating lab tests and screenings--at no out-of-pocket cost to you. Clinical studies suggest that receiving recommended screenings and identifying conditions early are critical to preventing chronic conditions or catching problems at the earliest, most treatable phase. A wellness exam is like a "tune-up" for the body--it's preventive maintenance that can help you avoid bigger problems later.

To use your HealthFlex wellness exam benefit, ask your physician or physician's office to code your exam under "wellness" or "preventive care."
