This memo is to inform you that Amgen and all divisions have access to fee waived Hertz #1 Club Gold enrollments via online enrollment. The instructions for users to follow are:

-Click CONTINUE.

--Next will appear the fee waived enrollment application.

-Fill out all the fields on the application for USA/Canada gold service. Please note: The credit card billing address listed for your form of payment must match identically to the preferred mailing address listed on the application.

-Enroll online by clicking CONTINUE.

-Check the Enrollment Confirmation Preview. If all correct, click I AGREE.

-Print the Enrollment Confirmation and make note of your #1 Club gold #.

-For Gold service outside the USA/Canada, the screen will ask: WOULD YOU LIKE YOUR #1 CLUB GOLD PRIVILEGES EXTENDED TO EUROPE, THE MIDDLE EAST, AFRICA, AUSTRALIA AND NEW ZEALAND?

-If so, click YES.

-Following the online instructions for registering for International rentals.)

Hertz will send a confirmation e mail within 24 hours. You may make a reservation using your new #1 club gold number once the confirmation e mail is received. Due to cost cutting, Hertz no longer mails gold cards. Hertz recommends you log on to and establish a password. Then click My Account and the option to print your membership card is available.

NOTE: Due to 9/11, a security measure has been added. All first time Gold reservations rental pick ups require that you present identification at the Gold Club counter.

DO NOT APPLY FOR GOLD MEMBERSHIP AT AS THIS IS NOT A FEE WAIVED APPLICATION. FOR ANNUAL FEE WAIVED, MUST APPLY AT WWW2.GOLDCORP.
