The purpose of this checklist is to ensure that you have reviewed the necessary enclosed information, completed the required forms, and submitted them to the correct locations.

"Dog Walk" Information and Pricing Sheet

"Dog Walk" Form

The form is only required if you wish to pay at the "Dog Walk" event by cash or check. Please note that payment for FHS

merchandise and PTSA bulldog apparel are separate. If paying by check, two checks will be required. For Folsom High School merchandise

(yearbooks, PE clothes, sports pass, student parking, and ASB sticker), please make checks payable to Folsom High School. For PTSA

bulldog apparel (sweatshirts, t-shirts, pajamas, stickers, bulldogs, etc.), please make checks payable to FHS PTSA.

Dress Code Policy

FCUSD ~ Instructional Calendar

Please review instructional days. Note: First semester finals are scheduled on December 18th, 19th, and 20th. Second semester finals are

on May 29th, 30th, and 31st. These are minimum days for students ~ 8:00 a.m. ? 12:23 p.m. Please do not schedule vacations during this

time. Final examinations are "mandatory" at Folsom High School and will not be given early for any reason. Students who miss their final

exams will be issued a grade of "incomplete" for the semester. Students are required to make-up their missed final exams during the first 2

weeks of the following semester to replace the "incomplete" grade with a letter grade. Failure to do so may result in a semester grade of "F"

(fail).

Please review, sign, and return the form to the checkout table at the "Dog Walk" event or turn it in to the Folsom High School

Administration Office during the first week of school.

Folsom High School ~ Parent Teacher Student Association (PTSA) Membership/Volunteer Form

Be a part of Folsom High School's Parent Teacher Student Association by completing the PTSA Volunteer/Membership Form. The PTSA

offers scholarships; however, membership is required for eligibility. Please turn your membership form in at the "Dog Walk" event or to the

Folsom High School Administration Office. For the PTSA website, please visit: .

Food Services Questions (no attachment)

We encourage you to complete the application for the Free and Reduced Meal plan. Please visit the Folsom Cordova Unified School District

website: ; select "Departments"; select "Food Services"; select "Apply Online Free and Reduced Meals" and complete the

application process. If you have any questions, please contact the Food Services Department at 916-294-9011.

The district policy on any and all medication(s) taken at school requires signed consent. Please review the policy, complete and bring the

necessary forms along with the medication(s) to the Health Services table at the Dog Walk event.

Transportation (no attachment)

If your son/daughter will be using FCUSD transportation, please refer to the Transportation Department for an application and information.

Applications are available through the Folsom Cordova Unified School District website: ; select "Departments"'; select

"Transportation"'; select "Forms"; then download the application. Applications will also be available at the "Dog Walk". Applications must

include a photo and a check or money order payable to FCUSD, and must be mailed to FCUSD Transportation Services, 11458 Elks Circle,

Please complete the following steps:

1) The athletic registration process is completed online. The Sports Physical form will need to be signed by a physician and then uploaded on the athlete's online athletic registration.

2) The athletic registration process must be completed prior to any summer or offseason workouts, camps, games, conditioning and/or weightlifting.

3) Sport physicals are valid for one calendar year. For example, if your physical form is signed on February 1, 2017, it will expire on February 1, 2018. The Sport Physical form must be signed by a medical doctor; chiropractors and physical therapists cannot sign a physical form.

To access the online registration and sports physical form, go to ; then click on the "Registration" tab. The Sports physical forms are also available in the FHS administration office.

For tryouts, please dress in workout clothes (shorts, t-shirt, athletic shoes), bring plenty of water to drink, and sunscreen is advisable.

Folsom High School PTSA Direct Appeal Fundraiser

This fundraiser is in lieu of sending students home with the task of selling door-to-door, collecting money, and delivering goods. We hope you will help us avoid that by supporting your PTSA with your donation and helping us achieve fundraising goals necessary to support students and faculty.

Our goal is to raise $100 per student. Every dollar helps and we appreciate all donations.

We strive to achieve 100% parent participation and support in order to maintain the benefits that the PTSA provides the school. Grandparents and other family as well as friends can donate, too!
