Freedom High School PTSA 2016 ScholarshipThe Freedom High School PTSA is proud to offer scholarships to graduating seniors planning to enter and attend a college, university or technical school in the fall following graduation. The PTSA Scholarship Review Committee will choose applicants based on eligibility requirements and completion of this application. Members of the Review Committee shall remain anonymous. The Committee will read and score all applications and essays resulting in the selection of four (4) candidates as recipients of the Freedom High School PTSA Scholarships. Four scholarships will be awarded in the amount of $1,000.00 each. One of these (4) scholarships is separately designated as the Cheryl Bacak Scholarship Award and will be awarded to a student who has demonstrated excellence in service and/or leadership. An addendum page is attached to this application, and a separate essay is required for consideration for this scholarship award.The scholarship funds must be utilized for school expenses only. The PTSA will prepare scholarship checks written directly to the institution.If a recipient fails to complete his or her obligations as outlined, the unused scholarship allocations and/or forfeited funds will remain the property of the Freedom High School PTSA.Please complete the entire application, place in a sealed envelope to the attention of the PTSA Scholarship Review Committee and submit to Khristie Greiner in the FHS Career Center no later than Friday April 8, 2016. NO exceptions will be made and all decisions are final. The four scholarship recipients will be announced at the Senior Awards Ceremony.Applicant Criteria:Applicant or their parent/guardian must be a member of the Freedom High School PTSA by February 26, 2016.Applicant must have attended Freedom High School for at least their entire senior (12th grade) year.Applicant must be a graduating member of the 2016 senior class of Freedom High School.Applicant must be bound for an institute of higher learning to include college, university or technical rmation requested to complete the application:Part I.GPAList your current GPA. Your Guidance Counselor MUST initial/verify this section.Part II. Student ActivitiesList all student activities in which you have participated during your high school years, including clubs, organizations, sports, etc.Part III. Leadership PositionsList any leadership positions you have held during your high school years; for example, an officer of a club or an organization, team captain or co-captain, editor, etc.Part IV. Special Awards, Recognitions or HonorsList any special awards, recognitions or honors received, including honor society memberships.Part V. Community InvolvementList community involvement, which includes but is not limited to: volunteer work, employment, church, scouts, etc. outside of school.Part VI. Additional InformationList any significant experiences or accomplishments, additional education, interests or other information you would like considered. Part VII. EssayThe essay should be typed on a separate sheet of paper (one page only, double-spaced, 12-point font). Do not include your name on the essay page.Part VIII. Continuing EducationApplicants must be bound to attend an institute of higher learning.
