How To Get Started

After you've completed your 2015 Annual Wellness Visit (if you're new to Medicare, this is called your "Welcome to Medicare Visit"), Highmark will send you instructions on how to claim your first Passport Reward.

Rewards Made Simple

We want to reward you for taking steps toward better health. In future reward periods, you'll be provided a selection of 3 healthy steps that can earn you Passport Rewards. Each time you complete a healthy step during the current reward period, Highmark will let you know and give you the chance to choose one of the available Passport rewards, which will then be mailed to you.

More About Your Passport Rewards

The Passport Rewards Program is managed by Highmark's Government Stars organization in accordance with the guidelines set forth by the Centers for Medicare and Medicaid Services. Participation in the program must be offered to all Highmark Medicare Advantage members without discrimination.

Members must complete either an Annual Wellness Visit or a Welcome to Medicare Visit to gain entrance to the 2015 Highmark Passport Rewards program and to be eligible for rewards offered in future periods.

Confirmation of successful completion of reward activities will be identified through the Highmark claims and billing systems, unless otherwise identified by Highmark. To earn a reward, the activity must be complete (i.e. have a service date) within the reward period window.

Claims processed after the reward period window closes will qualify for a reward if the service date falls within the reward period window. Highmark will notify members of reward opportunities and earned rewards based upon the rules of the program. Highmark reserves the right to adjust the reward options and value for each period. Limit: one reward per period.
