How to create your Sharecare account (have your Highmark member ID ready!):

Visit mycare. on your computer or mobile device.

2 Click on Create an Account near the bottom of the page.

3 Enter your information exactly as it appears on your Highmark member

ID card.

4 Agree to the terms of use and authorization.

5 Click on Create an Account.

Important tip: Whether using a computer or mobile device, you must visit mycare. to register for the first time. You cannot register for the first time on the Sharecare app.

How to complete the 2020 Wellness Rewards Program

To complete the program, earn 1,000 points by completing the following activities by 4/30/20:

Complete the Sharecare RealAge Test (250 points)

After creating your Sharecare account by following the steps to the left, take the Sharecare RealAge Test in your account.

Register for EdLogics (Self-Certify) (250 points)

Visit acshic.account/register or, if you already have an EdLogics account, visit acshic. and login using your existing username and password. Once registered, self-certify within your Sharecare account.

Register for Healthcare Blue Book (Self-Certify) (250 points)

Once registered, self certify in your Sharecare account.

Complete at Least One Preventive Activity From the List Below (250 points)*

Preventive Exam (250 points) Colorectal Exam (250 points) Breast Cancer Screening (250 points) Cervical Cancer Screening (250 points)

Note - If you already received a preventive exam/screening after January 1, 2019, you will automatically earn credit for that activity.

*Exams and screenings are claims based activities. Once your provider submits your claim, allow 4-6 weeks for your credit to display in your Sharecare account.
