|SUBJECT: CERTIFIED HOME CARE AIDE |

POSITION SUMMARY:

Administers personal care and assistance with activities of daily living for patients in the home setting under physician’s orders, assignment and supervision provided by the Registered Nurse or Physical Therapist.

EDUCATION AND EXPERIENCE REQUIREMENTS:

1. A recognized (State) Home Care Aide certificate obtained by completion of a Home Care Aide Training program and supervised practice.

2. Two years of experience as a Certified Nurses Aide and prior homecare experience is preferred.

GENERAL REQUIREMENTS:

1. Must have genuine interest and concern for all clients.

2. Must be sufficiently mature and emotionally stable to work well with others.

3. Must possess good physical and mental health.

4. Must have a clean and neat appearance.

5. Must be courteous and kind in manner.

6. Can work harmoniously and skillfully with other staff members, patients and their families.

7. Must be flexible and able to tolerate change or changing demands.

8. Valid driver’s license and access to an automobile.

ENVIRONMENT:

The Home Care Aide travels to and from, and performs her duties in, each patient’s home. The climate is not routinely controlled. Frequently there is exposure to blood, body tissues and other potentially infectious fluids. There can be exposure to loud and unpleasant noises, unpleasant odors, unclean homes and dust. May be subject to hostile and emotionally upset patients, family members or visitors.

PHYSICAL REQUIREMENTS:

1. Able to speak, read and write in English.

2. Able to read assignments, directions, labels and measure vital signs, including; temperature, pulse, respirations, blood pressure and weight.

3. Able to communicate and respond clearly on telephone and respond to patients spoken needs.

4. The ability to physically transfer, lift or assist patients whose average weight is 160 pounds with or without the aid of mechanical devices.

5. Able to spend 80% of the work shift standing and/or moving about.

6. Able to carry full bath basins, trays and position patients.

7. Able to walk, climb stairs, stoop, twist, bend and squat to perform essential job functions.

MENTAL REQUIREMENTS:

1. Able to concentrate on detail with frequent interruptions.

2. Able to follow, complete and remember daily routines and requirements.

3. Able to comprehend and utilize professional education materials.

4. Able to enact emergency procedures.

5. Able to make independent decisions when circumstances warrant such action.

6. Able to cope with the mental and emotional stress of the position.

EQUIPMENT:

Must be able to use: sphygmomanometer, stethoscope, adjustable and standard beds, side rails, gait belt, hoyer lift, scales, thermometers, watch, bathing equipment, water faucets, wheelchairs, walkers, hearing aides, electric shavers, catheters, bed pans, telephone.

DUTIES AND RESPONSIBILITIES:

1. Administer or assist patient with personal care including bath or shower, hair, skin, mouth, and nail care.

2. Monitor and records vital signs.

3. Assist patient with elimination using bathroom, commode, bedpan, or urinal.

4. Assist patient with transfer activities.

5. Assist patient with ambulation.

6. Assist patient with prescribed exercise after instruction by the Registered Nurse or Physical Therapist.

7. Assist patient with dressing or undressing.

8. Assist with household services that are essential to the patient’s home health care.

9. Report pertinent observations of patients’ physical and psychosocial status to Registered Nurse, Therapist or Home Health Director.

10. Document accurately and concisely all care and observations.

11. Submit accurate reports of patient visit charges, hours of work and mileage.

12. Adhere to hospital and departmental policies and procedures.

14. Consistently report for duty on time and as scheduled.

15. Observe appropriate safety measures.

16. Maintain strict confidentiality.
