Service Objective The Home and Community Care Program for Younger People (HACC-PYP) aims to provide an integrated range of basic maintenance and support services to eligible clients to:improve and maintain the client’s independence and capacity to live safely at home and participate in community activities and strengthen the capacity of the client’s carer to maintain their care role. 2. Description of the serviceProfessional nursing care including assessment and direct clinical care, education and advice and supervision designed to improve the client’s capacity to independently manage everyday activites, manage chronic disease and attain or maintain good health, mobility and safety at home. The support can include supporting the carer in the care relationship and training and supervising staff who provide personal care.Assistance is provided in a manner which promotes skills development, capacity building and independence.3. Client groupThe client group for this activity is people aged under 65 years (or under 50 year for Aboriginal and Torres Strait Islanders) who have difficulty performing activities of daily living and their carers who require the support of a professional nurse to address health related issues.4. Obligations specific to this activityIn addition to the obligations listed in the Service Agreement, organisations funded to deliver this activity must comply with the following:4a. Registration and AccreditationOrganisations that only receive HACC-PYP funding are required to meet the Home Care Common anisations that are also funded to deliver the Commonwealth Home Support Programme (CHSP) are subject to review against the Home Care Common Standards. From 1 July 2019, the standards will be replaced with the Aged Care Quality Standards and reviews will be undertaken by the Aged Care Quality and Safety Commission. Quality reviews for CHSP satisfy the HACC-PYP quality requirements.Nurses employed by the HACC-PYP funded organisations are also required to meet relevant professional registration and standards.4b. Program requirements and other policy guidelines Victorian HACC program manual 4c. Service redevelopmentThe department will continue to monitor service provision, and where necessary, may amend service specifications or funding allocations based on community needs or in response to changes to government policy and priorities.During the current Service Agreement period, the roll out of the National Disability Insurance Scheme (NDIS) will completed in Victoria. Community service organisations are expected to work collaboratively with the department and the National Disability Insurance Agency to facilitate a smooth transition. 5. PerformanceFunding is subject to achieving the performance targets specified in Schedule 2 of an organisation’s Service Agreement. Performance is measured as follows:Key performance measure 1: Number of hours of serviceAim/objectiveThe objective of this measure is to record the hours of nursing support provided to individual clients or carers.TargetThe performance target is provided in the Service AgreementType of count FORMCHECKBOX Cumulative FORMCHECKBOX Non-cumulative Counting ruleThe HACC MDS is used to record the hours of direct and indirect service to clients or carers.Countable time includes time spent: in contact with the client, their family or carers writing case notes attending case conferences monitoring and reviewing plans making referrals training of staff to support clientsData source(s) collectionHACC Minimum Data Set Definition of termsAn hour of service comprises time spent face to face and indirect time spent on behalf of the client or carer such as phone calls and time spent writing care and treatment.Funded organisations are requested to liaise with their departmental program advisor to confirm their 2019-20.performance target.6.
