Children's Waiver Amendment and Approval

Rationale ? In 2020, there were numerous discussions with Medically Fragile care management agencies,

providers, Medicaid Managed Care Plans, and Health Home surrounding the needs of the population ? Stakeholders provided feedback and examples of the needs of caregivers and family members to

assist and prepare for life-threatening / terminal diagnosis of their child/youth

Implementation ? NYS DOH was expecting that the policy/changes in this webinar would take effect September 1,

2021, however the Centers for Medicare and Medicaid Services (CMS) has not yet approved the Children's Waiver Amendment submitted April 2021 ? NYS DOH will make an announcement and issue the Bereavement Counseling and Health Home Services policy once approved by CMS ? We are looking forward to an October 1, 2021, approval date
