Please complete the tables below with relevant information about participating team members and associated organizations. Participating states should identify up to six representatives who will comprise the state’s cross-sector team and who will consistently participate in learning collaborative activities. Project team membership could include, but is not limited to:Senior leadership from executive branch agencies (e.g., Medicaid, Aging, Veterans, and/or other Health and Human Services divisions); Leadership from Health, Aging, or other relevant Legislative committees; Senior representatives from governors’ offices; Representatives from community-based or private sector organizations (e.g., state chapters of AARP or the Alzheimer’s Association, family caregiving advocacy organizations, Area Agencies on Aging, and/or universities). In terms of time commitment, we anticipate each state team spending two to three hours per month on this project, which may include a monthly or bi-monthly technical assistance call with CHCS, an all-state webinar, and/or other learning collaborative opportunity. Please describe the current policies or programs in place in your state that are designed to support family caregivers. If your state has several efforts in place, please describe the most prominent, and/or those for which you can describe measureable progress or outcomes. If your state is in the early stages of family caregiving policy and program improvements, please describe the overarching vision and priorities for improving family caregiving in your state. Choose an Area of Focus. Please review the list of Areas of Focus below and choose one that best aligns with your state’s interests regarding family caregiving. All project states will develop action plans, goals, milestones, and outcomes within their chosen Area of Focus. ?Strengthening family caregivers’ capacity, including, for example, through use of new technologies, increased access to respite care, and formal training for family caregivers?Establishing robust data collection strategies1 to enhance programs or inform policies to support family caregivers (e.g., family caregiver needs assessments, surveys of program effectiveness) ?Building formal cross-sector partnerships (e.g., across aging, housing, transportation, health plans, etc.) in a deliberate and strategic way to better support family caregivers’ needs?Developing family caregiver supports within statewide aging initiatives through formal linkages to prioritize family caregiving (e.g., within state taskforce focusing on aging or age-friendly health systems or communities broadly).With the Area of Focus selected, please describe one to two initial ideas, policies, or programs your team may want to engage in over the course of the project that align with that focus. Leadership involvement and support. Please describe the level of Executive and/or Legislative support in your state for family caregiver-focused initiatives. If possible, please describe specific leaders who could support your team throughout the course of this project. If none are explicitly known at this time, please indicate any leaders your team may want to cultivate.Data collection plan. Please describe how you already or would plan to collect and use data during the project period, including any systems or processes already in place. Please identify two to three specific indicators you could use to assess progress toward your priority area goals, and data elements you could use to measure movement across those indicators. Family caregivers in communities of color. Please describe how your team’s efforts could specifically help family caregivers in communities of color. What are the needs of those populations in your state? How would your work address these needs? Challenges to Participation. Please briefly list any potential challenges your state may face that may hinder your participation in this project, including challenges created by the COVID-19 pandemic. What strategies could be put in place to mitigate risks to project participation? Letter of Support Applicants must obtain a letter of support for participation from a member of their state’s Executive or Legislative leadership team (e.g., governor, Medicaid director, state representative, aging director). The letter should reiterate that family caregiving is a state priority and highlight state leadership’s support of the project team’s area of focus and participation in learning collaborative activities. ................
