For online submissions requesting payment in the form of a Mastercard Prepaid Card, payment will be delivered via email with instructions for claiming your Prepaid Card within 8-10 weeks after receipt of the completed rebate form and copy of original invoice. You must claim your card within 3 months from the date these instructions are sent via email. Your right to the payment expires after that time.
