JOB DESCRIPTION AND PERSON SPECIFICATIONJob Title:Day Hospice Team Leader (Senior Nurse)Reports to:Responsible for:Head of Clinical ServicesDay Hospice Nursing TeamLocation:Primrose Hospice St Godwalds Road, Bromsgrove B60 3BWHours:22.5 hours Per week (over 3 days, currently Wednesday, Thursday and Friday)Salary:?20,400 (?34,000 FTE)Job RoleThe Day Hospice Team Leader will:Work as team leader overseeing Primrose Day HospiceBe responsible for assessment and planning of care for patients attending Day HospiceProvide specialist advice to the Day Hospice clinical team and other health care professionalsBe responsible for staff management including supervision, appraisals, rotas, recruitment and selectionDrive utilisation of Day Hospice ensuring resources are effectively utilised and patient benefit maximisedUndertake the day to day management of Day HospiceDevelop a programme of in-house education for staff and volunteersDuties and ResponsibilitiesClinical Receive referrals from other professional disciplines, assess and effectively prioritise these in order to effectively utilise the resources of Day Hospice.Undertake assessments of palliative care needs and work with primary and secondary care givers to improve the quality of life of patients, relatives and carers living at home with life-limiting illness. Some assessments may take place in the patient’s home.Use advanced communication skills to communicate highly sensitive information to patients and carers e.g. breaking bad news, end of life discussions.Provide nursing and personal care as required, including assisting dependant patients with bathing and toileting.Act as a resource to all health professionals caring for Day Hospice patients, giving specialist advice regarding complex symptom management and care planning.Develop and maintain effective communication with all members of the multi-professional team, community services, statutory and voluntary agencies.Ensure all activities are consistent with Primrose Hospice policies and procedures.Maintain accurate and up-to-date patient records using electronic databases.EducationPromote an environment conducive to training needs of all staff, volunteers and visiting students.Promote the use of evidence-based practice.Develop a programme of in-house training for staff on aspects of palliative care.Provide education and training for patients, relatives and carers.Participate in local and county-wide palliative care education initiatives and development.Provide placements/mentorship for professionals undertaking palliative/oncology courses at graduate/undergraduate level.ManagerialFoster a sensitive and caring environment for patients, relatives and staff.Manage the clinical team within Day Hospice, exercising leadership by example and developing a management style which encourages individual growth and promotes high anise Day Hospice by prioritising duties, allocating staff, supervising the work of qualified and unqualified staff and ensuring that care given is both safe and appropriate at all times.Manage, appraise and develop members of the nursing team within Day Hospice. On an organisational level, manage the activities of subcontracted services including Occupational Therapist, Physiotherapist and Therapies Assistant.Support the Head of Clinical Services, maintaining continuity and consistency of management and leadership in line with policies agreed within the Hospice.Develop clinical protocols relevant to developing service in Day Hospice.Maintain effective communication between all staff including volunteers within the Hospice. Attend meetings as required with all members of the multi-professional team.Review current practice, contribute new ideas and co-operate with changes in policy as they occur.Maintain accurate records and prepare reports and statistics as required for Board and Committee meetings.Assist with the orientation of staff new to the Hospice.Liaise with support services (catering staff, contract cleaners, volunteers etc.) to ensure that satisfactory levels of service are maintained, bringing any deficiencies to the attention of the Head of Clinical Services.Maintain appropriate levels of stocks and supplies within Day Hospice, ensuring that the unit is always adequately stocked, with due regard to economy and efficiency.Take appropriate action on any accident or untoward incident and report and record details of such events as soon as possible to the Head of Clinical Services.Act as an ambassador for the charity at all times.Continuing Professional DevelopmentMaintain and develop specialist knowledge, skills and competence through networking with other specialists and professional organisations.Maintain a programme of professional development by attending internal and external training, education, conferences and seminars.Contribute to the assessment of personal learning needs and opportunities to inform and develop personal development plans.Participate in team clinical supervision.Professional ResponsibilityMaintain registration with the NMC, maintaining personal professional portfolio.Take personal responsibility for attendance at annual mandatory training sessions.Take personal responsibility for continuous professional development.Person SpecificationQualificationsEssentialDesirableRGNProfessional qualification/diploma or degree level or equivalentPost-basic qualification in palliative care or equivalent experienceRelevant short courses e.g. Symptom Control and End of Life CareWilling to undertake a non-medical prescribing courseEvidence of continued professional developmentSkills for the roleEssentialDesirableHave advanced communication skills and the ability to communicate effectivelyKnowledge of palliative careAble to motivate self and othersAble to assess, plan and deliver individual care in partnership with patients/their carers/other professionalsAble to work across the multidisciplinary health and social care teamAble to handle distressing situations with sensitivity and empathyIT skillsVenepuncture and/or cannulation skills (training will be given)Interest in working with volunteersBreaking bad news experienceKnowledge for the roleEssentialDesirableExperience of working at minimum Band 6 levelAble to demonstrate considerable experience in a nursing role managing complex patientsExperience of working within the field of palliative care/caring for palliative care patientsAble to demonstrate considerable experience in a nursing role whilst working with a multidisciplinary team; communicating effectively at differing levelsExtensive experience of team managementExperience of incorporating policy/evidence into practiceExperience of clinical auditExperience in developing or reviewing standards/policiesCommunity experienceOncology experienceOther dutiesTo undertake any other duty within your ability and within reason, as may be required from time to time, at the direction of your line manager. If a volunteer is assigned to assist you at any time, you will still retain responsibility for the requirements of this job in terms of accuracy, efficiency and standards of completion.
