Procedure Service/SOSHR Approval: Troy SchuhmacherDate: 5/20/2019JOB SUMMARYThe Clinical Program Coordinator (CPC) for the Hospitalist Procedure Service is a Registered Nurse who possesses expert clinical knowledge in the care of patients as well as bedside procedures performed by the Hospitalist Procedure Service. The CPC uses expert clinical knowledge to systematically and continuously collect data in collaboration with multidisciplinary members of the care team to provide therapeutic care with the patient and/or family. This position coordinates the needs of patients undergoing bedside procedures. The CPC has daily management responsibility for patient care including teaching patients, families, and staff about the procedure process as well as post procedure care, screening patients for risk factors prior to invasive procedures, ensuring that pre-procedure requirements have been met, following up after procedures as necessary, and maintaining excellent procedural documentation and database collections. The CPC works closely with a variety of Hospitalist Providers and inpatient and outpatient areas within University Hospital. The CPC also holds expectations of being able to competently perform in the Save Our Shift (SOS) role within the Float Pool. The CPC has a strong knowledge base and is competent to care for a diverse population of patients and families. They are responsible to practice within their level of competence for the safe delivery of patient and family care, are expected to keep current with changes in practice on the various nursing units as well as have a working knowledge of equipment used with patients.MAJOR RESPONSIBILITIESClinical Practice and Program Coordination Integrates the principles and philosophy of the professional nursing care delivery system into place. Serves as a role model to other nursing personnel by demonstrating effective direct patient care, leadership and professional behavior.Coordinates care with inpatient and outpatient areas to fulfill procedure requests. Provides direct nursing care and patient education with patients/families, assuring continuity of care.Serves as a liaison with medical staff to identify issues of mutual concern and solve problems.Enforces adherence to hospital and department policies and procedures.Screens patients referred for Hospitalist Procedure Service consults for risk factors for the procedure.Explains procedures to patients and family members. Verifies consent.Educates patients, families and staff about post-procedure care.Serves as the liaison with the Hospitalist Procedure Team and Nursing. Establishes an in-depth data base by assessing the behavioral and physiologic status of the patient utilizing interview, observation, physical examination, and other available data.Integrates nursing interventions into the multidisciplinary plan of care, incorporating appropriate standards of care and practice and patient outcomes.Sets priorities adapting to changing patient and unit/clinic situations. In response to data indicating urgent and/or emergent risk to patient, initiates action to reduce or correct risk.Assumes responsibility and accountability for effectively managing nursing care of individual patients as a team member.Documents the nursing process to reflect a comprehensive and integrated approach to patient care.Evaluates patient’s responses to care based on effectiveness of nursing interventions/actions in relationship to established outcomes and consults with Medical Staff, Care Team Leader, Case Manager, and/or Nurse Practitioner regarding clinical variances and recommended changes to the plan of care.Identifies learning needs with patients/families and teaches by adapting standard information to provide individualized and comprehensive teaching-learning. Utilizes the resources of the Teaching-Learning Center as appropriate to meet identified patient, family and/or caregiver needs.Demonstrates the ability to quickly assess a patient when there has been a change in status and help to monitor and provide intervention for the patient.Demonstrates excellent vascular access skills to ensure safe techniques used to prevent line infections, safely remove central lines, and gain access on patients with difficult vasculature.Transport of patients requiring monitoring during transport and procedure.Administers moderate sedation and monitor during and after procedure.Responds and provides support for multiple emergent situations; Blue Cart, Medical Response, Rapid Response, and traumasResponds and manages patients who have a change in condition as a member of the Rapid Response TeamManages patients on nursing units needed a higher level of care until appropriate transfer to a higher level of care can be facilitatedSupports the ED and other critical care areas when there is high volumes and increased acuity of patientProvide Leadership for Hospitalist Procedure Service GoalsServes as a role model, demonstrating effective patient care, leadership and professional behavior.Acts as a consultant and clinical resource to staff through provision of patient care and coordination of care.Coordinates medical and nursing management of patient care.Supports patient advocacy and the patient’s Bill of Rights.Develops, maintains, and evaluates patient care standards in collaboration with Hospitalist Providers and other organizational leaders.Promotes a department-based improvement program that incorporates patient care standards and meets The Joint Commission (TJC) guidelines.Education/OutreachServes as a resource to staff in other departments regarding bedside procedures.Interprets and supports the philosophy, objectives, policies and procedures which guide personnel, patients and the public.Participates in patient, family, and staff teaching.Meets personal continuing education requirements.
