Work week: Sunday- Saturday

For SIN 621-010, Physician, Neurologist:

Overtime/Holiday Pay Rate: No overtime – Physician receives the normal hourly rate.

Weeknight On-Call Rate: Weeknight is a one-time flat charge of one hour of the normal hourly rate for a 15.5-hour period when a physician is required to be available by phone or pager and be able to provide phone consultation relative to their specialty. The weeknight period is generally 4:30 pm – 8:00 am. Physician must be within 30 minutes or 30-mile radius from the facility.

Weekend On-Call Rate: Weekend on call is a one-time flat charge of one hour of the normal rate

for a 24-hour period on Saturday and Sunday when a physician is required to be available by phone or pager and be able to provide phone consultation. Physician must be within 30 minutes or 30-mile radius from the facility.

Call Back Rate: If the physician must report to the facility and see patients, the normal hourly rate applies when physically onsite for all hours worked. One-hour minimum charge applies for call back.

For Allied Healthcare Providers (SINs 621-034 and 036):

Overtime: 1.4x the normal hourly rate for all hours worked in excess of a 40-hour work week.

For Colorado: 1.4x the normal hourly rate for all hours worked in excess of 12 hours in any workday. Overtime in accordance with state and local labor laws.

Holiday Pay Rate (for all Federal Government Holidays): 1.4x the normal hourly rates applies for all hours worked in excess of 40 hour work week.

Weeknight/Weekend On Call/Call Back:

On Call: $8.00 per hour charge for the time the provider is on call. On call is to carry a cell phone or pager and be able to provide phone consultation relative to their specialty and must be within 30 minutes or 30-mile radius from the facility.

Call Back: The normal hourly rate applies if less than 40 hours worked. In excess of 40 hours, overtime of 1.4x the normal hourly rate applies for each hour worked when call back and physically onsite at the facility. One-hour minimum charge applies for call back.

Minimum Qualifications of Professionals:

SIN 621-010 – Physician, Neurologist

Education: Possess M.D. or D.O. with graduation from an accredited school of medicine. Completion of internship and residency in neurology.

Experience: Possess a minimum of one (1) year experience in their respective specialty within the preceding three years.

Licensure: A current valid, unrestricted license to practice in one of the 50 States, DC & PR

Certification: Board Certification by the American Board of Psychiatry and Neurology (ABONP), ACLS, BLS and other applicable certification/training required in task order.

SIN 621-034 – Registered Respiratory Therapist

Education: Graduate from an accredited respiratory therapy program.

Experience: Possess a minimum of one (1) year experience in their respective specialty within the preceding three years.

Licensure: Current valid, unrestricted license to practice in one of the 50 States, DC & PR

Certification: Certified as an RT, Commission on Accreditation of Allied Health Education Program (CAAHEP) or National Board for Respiratory Care (NBRC)

SIN 621-036 – Perfusionist

Education: Graduate from an accredited perfusionist program by the Commission on Accreditation of Allied Health Education Programs (CAAHEP). Bachelor’s degree.

Experience: Possess a minimum of two (2) year experience in their respective specialty within the preceding three years.

Licensure: Valid State license (depending on state)

Certification: Certified Clinical Perfusionist (CCP), American Board of Cardiovascular Perfusion (ABCP), CPR and BLS certification
