Requirement criteria for Regeneron Therapy Clinic

Must have COVID-19 positive test result in hand. Must be 12 years of age or older, and weigh at least 88 lbs. Please bring identification (and insurance card, if applicable)

Patient must additionally meet at least one of the following:

Our criteria has been adjusted due to federal and state shortages of the Regeneron medication supplies.

Post Exposure Prophylaxis patients will be limited to those who are transplant patients or on chemotherapy (this requires an order and must be scheduled; please fax orders to 478-975-6776, include the patient's phone number so we can call to schedule this appointment) Covid + Ages 12 ? 17: obesity defined as weight at the 85th percentile or greater for their age and gender (based on CDC growth charts) - OR - meet one of the standard criteria below. Covid + Age 18 and above: must also meet one of the standard criteria below.

Standard Criteria: Elderly (65 years or older) (BMI) > 25 Pneumonia diagnosis Pregnancy Chronic kidney disease Diabetes Immunosuppressive disease or immunosuppressive treatment Cardiovascular disease (including congenital heart disease) or hypertension

Chronic lung disease (e.g., chronic obstructive pulmonary disease, moderate-to-severe asthma, interstitial lung disease, cystic fibrosis, pulmonary hypertension) Sickle cell disease Neurodevelopmental disorders (e.g., cerebral palsy) or other conditions that confer medical complexity (e.g., genetic or metabolic syndromes, severe congenital anomalies) Medical-related technological dependence (e.g., tracheostomy, gastrostomy, positive pressure ventilation not related to COVID-19)

*In addition, other medical conditions or factors may also place individual patients at high risk, and authorization of Casirivimab + Imdevimab under the EUA is not limited to only those medical conditions or factors listed above. Health care providers are advised to consider the benefit-to-risk of an individual patient.

***Vaccination status has no bearing on eligibility to receive Regeneron therapies.
