If the patient is already on an oral opioid calculate equivalent parenteral quantity and enough for breakthrough doses i.e. a third of the 24 hour daily dose. E.g. Oral morphine: parenteral diamorphine 3:1 . morphine sulphate modified release 60mg b.d. = 40mg Diamorphine injection over 24hr and 5mg diamorphine injection for each breakthrough dose. ................
