Donate Life Missouri Registry

The Registry is a confidential and voluntary list of people who have chosen to be an organ, eye and or a tissue donor at the time of death. Regoster Today by comleting the online enrollment form. Register each time you visit your local drivers license office to obtain or renew a driver/non-drive license (all endorsements). Print, sign, and mail in a registration form to the address on the form. Call 888497-4564 if you do not have Internet access.

Living donation offers an alternative for individuals waiting for a transplant from a deceased donor. You can donate a kidney, lobe of a liver or lung to a recipient.
