Leave of Absence Process

Objective: To simplify administrative process for all leaves of absence to comply with Board Policy.

Request for Leave of Absence

Any absence that extends more than 5 consecutive days OR Illness that requires intermittent leave under the Family Medical Leave Act (FMLA). (This includes extended absences and intermittent absences due to Workers Comp) *If employee is returning on the 6th day after 5 days absent, a doctor's note should be provided to the immediate supervisor certifying an ability to return to work. 1. Employee completes Request for Leave Form (found on Employees Leave of Absence webpage): 2. Employee attaches, "The Certification of the Healthcare Provider"(Different forms for employee care vs. family

care) 3. Employee presents form to Supervisor/Secretary for notification to school 4. Supervisor forwards form and documentation to: Leah Gmerek, Leave Administrator

Notification

Human Resources will review the request, provide notification to employee and supervisor of eligibility, and determine if further documentation is needed and if approved with or without the use of sick days.

Communication during Leave

The Supervisor maintains responsibility for follow-up with an employee to determine their return-to-work date based on approval dates provided from Human Resources.

Failure to contact supervisor at the expiration of the doctor's certification (date the doctor says you should be able to return to work) may result in separation of employment, if employee does not report to work on the date of expected return to work or did not provide the required medical note for the district to consider an extension to the medical leave.

Request for Extension to a current Leave of Absence

Any leave of absence that extends beyond the initial date of approved leave/what the doctor had certified: 1. Employee utilizes original approved Leave of Absence Form 2. Employee indicates "Extension" ? see box under signature line 3. Employee submits form and doctor's certification or documentation to their Supervisor 4. Supervisor forwards form for Extension: Leah Gmerek, Leave Administrator

Return to Work following a Leave of Absence

Any employee returning to work following an approved Leave of Absence: 1. Employee completes Return to Work Form (found on Leave of Absence page) ? 2. Employee includes the doctor's note certifying the release to work with date included and if any

accommodations are needed upon the return to work. If there are work restrictions (A Minimum of Two Days needed for Review), HR and Supervisor reviews restrictions - prior approval is required prior to employee returning to work. Restrictions/Accommodations include: reduced schedule, mobility devices, lifting, stooping, bending, walking or reduced duties will need to be reviewed prior to a return to work. 3. Employee presents form to Supervisor with the doctors release 4.
