SUBJECT: Home Care Certified Nurses Aide - Job Description

POSITION SUMMARY:

Provide simple procedures as extension of therapy services, personal care, and household services according to the CNA Plan of Care. Follows written instructions of the Home Health/Hospice Registered Nurse or supervising Therapist. Provides emotional support to clients and their families. Documents services provided. Reports as appropriate to the HH/H Registered Nurse.

ESSENTIAL FUNCTIONS:

• Regularly attends Hospice IDG and Home Health Case Conference and actively participates in care planning/ coordination

• Provide services according to the CNA Plan of Care

• Observe, report to an RN and document patient status, and all care or services furnished accurately, concisely and timely

• Obtain and record temperature, pulse, respiration, and weight as directed by the CNA plan of care

• Follow basic infection control principles

• Observe basic elements of body functioning and report changes to the HH/H RN

• Maintain a clean, safe, and healthy environment in the patient's area.

• Follow emergency procedures as directed

• Provide to the patient appropriate and safe personal hygiene and grooming

• Provide to the patient safe transfer and ambulation as directed

• Provide nutrition and fluid intake as directed

• Assist with medications that are normally self-administered

• Provide Home making services as assigned

• Complete appropriate documentation timely

• Provides and maintains a reliable mode of transportation for all scheduled shifts, and have the ability to drive safely in all types of inclement weather

• Show empathy, respect and concern for, and preservation of the dignity of, all persons using the Service, all persons employed by the agency, all members of the public who are met while in any way representing the service

• Provide all care of clients without regard to race, color, sex, age, religion, illness, or national origin

• Fulfills responsibilities and duties required by Bonner General Health.

• Is willing to cross train to provide CNA Services in all Hospital Departments as needed

• Participates in/ manages daily and weekly scheduling for optimal, efficient patient care provision

• Responsible for establishing and maintaining healthy interpersonal relationships with all staff members

• Maintains strict confidentiality regarding knowledge of all patient information (HIPAA), staff and hospital confidential business

OTHER RESPONSIBILITIES:

BGH maintains the right to establish work schedules and to be ensured of employee’s attendance when scheduled. Maintaining regular attendance and punctuality is a required Essential Function of this position

Maintains licenses and certifications necessary to perform the job functions, and as required by State and hospital policies

BGH operates 24 hours per day, 365 days per year. The operational needs of departments and safety of patients may require that employees be available to work all shifts, weekends and holidays and in any department to meet the needs of the patients.

JOB SPECIFICATIONS:

• Skills:

• Demonstrates written and functional competence in all areas as required by CMS/Federal and State guidelines

• Ability to read, write, communicate in English

• Ability to carry out directions of RN or supervising Therapist

• Ability to note changes in conditions that should be reported

• Ability to promote personal hygiene and personal care

• Demonstrates basic understanding of principles of nutrition and meal planning, the aging process, and emotional problems of illness

• Successful staff will have an interest in maintaining and increasing health care skills

• Sympathetic attitude toward the ill, the dying, and their loved ones

• Commitment to work with Interdisciplinary Team members, other staff, and Hospice volunteers

• Ability to deal effectively with the emotional and physical demands of the job

• Vehicle operation in inclement weather

• Willing to cross-train to other hospital departments as requested.

• Education:

• Successful completion of a Certified Nurses Aide course and Hospice Training

• Demonstrates competence with Home Health aide certification with orientation period

• Certified by the State of Idaho and listed on the Idaho State Board Registry

• Professional experience working with those who are dying and those who are grieving is preferred

• Willingness to attend available inservice education workshops, seminars and programs regarding patient care. (Twelve hours per year are required)

Current CPR certification

← Personal vehicle for on-the-job transportation in all weather / road conditions / climate

← Provide proof of liability insurance on personal vehicle

← Must be able to successfully pass a driver’s license record background check

← Employment is contingent upon successfully passing a fingerprint-based criminal history background check through the Idaho Department of Health and Welfare

• Job allows employee to vary physical position or activity for comfort
