Tardies,likeabsences,interrupttheschool's efficiency. Timeislost,classesaredisruptedand anunnecessaryhardshipisplacedonteachers andstudents. Theattendancecoordinatorwill contactparentsonadailybasisandoccasionally ahomevisitwhennecessary. Excessive absencesaregroundsforreferraltothenetwork supportandinsomecasesoutsideagencies. MTSS:Multi-TieredSystemsofSupport ParkManorwillcontinuetoenhancethe disciplineprogramandsocialemotional competenciestoenhanceandenforceschool regulations,relatingthemtosystemwide policies.Allteacherswillsubmitandimplement aclassroommanagement/behaviormodification plan.

HealthRecords Allstudentsenteringkindergarten,sixthg rade, 8thgradeand/orwhoarenewtoParkManor Schoolmusthavetheircurrentmedical,dental andimmunizationrecords(includingproofofthe HepatitisBshots)c ompleteup-to-datebythe firstdayofschool.Theserequirementsare mandatedbytheStateBoardofHealth. As statedbytheStateofIllinoiskindergarten, secondandsixthgradestudentswillberequired tohaveadentalexam. A nynon-compliancewill resultinexclusiono fthec hildfromschool. Medication Onlythosemedicationswhichareabsolutely necessaryforthecriticalhealtha ndwell-beingof astudent,asdeterminedbythestudent's physician,duringschoolhoursmustb esubmitted ontheChicagoP ublicSchoolsmedication requestform. Theseformsa rea vailableinthe nurse'soffice. Theschoolmusthaveonfilea

letterfromthephysicianandparentrequesting theschooltoadministerthemedication. Once theformsareo nfile,themedicationsmustbe broughttoschoolbyaparentorlegalguardian. Parties&Nutrition DuetotheCPSHealthierSchoolDayInitiative, NOOUTSIDEFOODORDRINKSa re permissibleforconsumptionordistribution duringschoolhours. Onlysnackspre-approved bytheCPSNutritionPolicywillbedistributed (bystaffonly)tostudentsduringcelebrations. Studentsareallowedtobringapersonallunch; however,thelunchmustbeabalancedmealand beasingleservingsize.Studentsarenotallowed tosharetheirpersonallunchduetohealth concerns(i.e.allergies,restrictions,etc). Excessivesnacks,candiesandsodaswillbe confiscatedbyParkManorstaffandreturnedto thestudentupondismissal. FieldTrips Afieldtripthatcorrelateswithacurriculumarea maybescheduledforyourchild'sclassduring theschoolyear. Eachchildwhoparticipatesin thefieldtripmusthaveasignedpermissionslip. Parentsareaskedoccasionallytochaperone classesonschool-relatedfieldtrips. To chaperoneafieldtrip,youmustbeanapproved CPSvolunteer.ParkManorSchoolandCPS regulationsdonotauthorizeyoungersiblingsto ridebusesorattendschoolrelatedfieldtrips. UseofCellPhones Onlyuponwrittenrequestfromaparentor legalguardian,theprincipalmayauthorizea studenttopossessacellulartelephonefor medicalandotherfamilyemergencies,orfor anyothergoodcause. Primaryand IntermediateLevelstudentsthatareapproved mustsecuretheircellphonesintheirbookbags orfollowtheteacher'sclassrulesforc ellphones (includingsubmittingthephonetotheteacher dailyuntildismissal). MiddleSchoolstudents mustsecuretheircellphonesintheirlockers. Forstudentsthatdonotcomplywiththerule, thephonemaybeconfiscated. Ifconfiscated, aparentmustcometothemainofficetopick upthephoneMONDAYafternoonbetween 2:00pm?2:45pmonly. Repeatedoffenseswill
