HUD IDIS FORMSCDBG-I InstructionsImportant Information:REPORTING TO HUD: ACCOMPLISHMENTS & BENEFICIARIESHUD IDIS FORM Accurate and Complete Reporting is RequiredLocal government grantees are required to report all accomplishments achieved for each activity in a program year, excluding planning and administration activities. Complete information must be reported for each activity. These data are required to demonstrate national objective compliance and to account for the use of CDBG funds. The data required for each activity are dependent on the activity’s matrix code and national objective. The matrix code identifies the eligible use of an activity and is required for each activity in IDIS. Each activity, except for planning and administration activities, must meet a national objective to either benefit low- and moderate income persons, aid in the prevention or elimination of slums or blight (SB*), or meet an urgent need (URG). Housing Connections (Hook Ups): Direct Benefit Data by HouseholdsFor Low/Mod Housing (LMH) activities with an accomplishment of Households or Housing Units, the race/ethnicity and income levels data is reported by households. Count each household as one, regardless of the number of persons in the household. Household means all the persons who occupy a housing unit. The occupants may be a single family, one person living alone, two or more families living together, or any group of related or unrelated persons who share living arrangements. Infrastructure: Area Wide Data by PeopleFor Low/Mod Area (LMA) activities with accomplishment of People, the race/ethnicity and income levels data is reported by People.
