Overview: Performance Form
Getting Started: On the Home Page, you'll see a To-Do tile for any
modules that have pending tasks assigned to you. Either (a) Click on the

hyperlink to navigate to your nurse evaluation form or (b) Click the next to Home to view your menu options and select Performance.
You'll land on the Evaluations tab and will see a list of forms available to
you.
Performance Evaluation Action Buttons: ? Click the icon to enable spell check, legal scan, or view information
for the form.
2 ? Click the icon to print the evaluation.
? Click the icon to save the evaluation as a PDF.
? Click the icon to save the form. Note: The system autosaves the form every 15 to 20 seconds.
Notification Tiles: 4a) Displays any incomplete items. 4b) Displays the rank order of overall performance scores for all direct reports, with the same form.
3 4c) Click on the tile to view feedback that was submitted using the Get
Feedback function. Any feedback provided will also be viewable within the form at the appropriate place. 4d) Displays the number of items where there is a gap between the Leader and Team Member's ratings.
Performance Evaluation Sections: Select an option to jump directly to that
4 section of the performance evaluation form. Note: The section you are in will be
bold and highlighted.
Route Map: The steps in the performance evaluation process are listed here.
? - The current step is green. Remember to check here to see if the step
has a Step due Date.
? - All completed steps will have a small green check to the right of the
step number.
? - The group icon indicates multiple people may be involved in a step,

for example, when users may be completing their self-evaluation or providing feedback.
? - Click the information icon to see a description of the step, including
responsibilities and the participants involved in the step.
?
- An Action button will be visible if you need to move the form to
the next step. Click on the button to see the options.
Note: Click on the arrows on the left or right of the route map to view more
steps.
Note: In Step 2, Leader may begin drafting the evaluation without ratings/comments being visible to
the Team Member, and the Team Member should complete their self-evaluation, without having to
send form back and forth to each other. However, they cannot access the form at the same time. When
exiting the form, each user must click "Save and Close," otherwise the other user must wait 60
minutes before accessing the form. In step 3, Leader may send form back to Team Member to edit
self-evaluation, or to other Leaders to collect Feedback, but Leader cannot access form again until

user returns it to the Leader.

Evaluate Organizational Competencies
Competency Ratings: Select a rating for each competency based on

observable behavior. Click the down arrow to show a list of the rating choices. Click the icon to see a definition of each rating in the scale.
Note: Different sections of the form may have different rating scales.
Competency Comments: Enter comments based on observable behavior. You can format the comments and include a list or highlight with the
2 available Rich Text Features.
? Click on to spell check your comments. ? Click on to complete a legal scan.
Writing Assistant: Click the Writing Assistant icon to view suggested quotes specific to the competency you are rating. Choose a behavioral statement from one of the columns (Improvement Needed, Good, Excellent
3 Outstanding) that best describes your performance. Click on Place Quote
to add the comment to your performance evaluation. Click Close to cancel or when you are finished adding quotes. Note: You can edit the quote once it has been placed in the comments box.
View Team Member Ratings & Comments: The ratings and
4 comments provided by the Team Member in the self-evaluation will appear
here.

Evaluate Job Specific Competencies
Competency Ratings: Select a rating for each competency based on

competency performance. Click the down arrow to show a list of the rating choices. Click the icon to see a definition of each rating in the scale.
Note: A * indicates a rating or comment is required.
List Accomplishments: Comments are required. Enter a description of the work accomplished during the performance evaluation year related to job/position specific competencies. Your comments should describe what
2 was done and how it impacted the Network, Medical Center, department, or
unit. You may apply Rich Text Features to format your comments and conduct a spell check or legal scan to check for potentially inadvisable words.

Verifications: Click on the next to each question and select the appropriate response. These are required to be answered by the Leader.

Performance Rating Summary
Overall Performance Rating: The overall rating is visible along with the
1 unrounded rating. Note: If you make any changes to a rating, click Save to
recalculate.
Competency Ratings: The rating by competency is displayed. Click on the name
2 of a competency or the section name to quickly navigate to that section to review or
edit the rating and add or revise comments.
Overall Summary Comments: Enter comments for the Team Member's overall
3 performance. Note: Comments are required and spell check and legal scan are
available.
Action Options: ? Save and Close to return to the form and complete later. ? Get Feedback to solicit comments from another Team Member or Matrix
Manager [see right for instructions].
4 ? Send to Team Member to allow them to do, edit, or update their self-
evaluation (they won't be able to see your ratings/comments at this point). ? Send to Next Step (Executive Review/Calibration) to forward the form
when you are satisfied with all of the content. Note: You'll need to click Send to Next Step one more time to confirm and officially submit the form.
Get Feedback
Get Feedback: Click on Get Feedback from the bottom of the
1 form OR click on
from the route map at the top of the
form.
Search by Filtering: A pop-up window will appear to search using filters or by name. Click on the drop down arrow to reveal the
2 options and set the filters, then click on Search to provide a list of
names that match the criteria. Click on the radio button by the name and chose Select [not shown].
Search by Name: Begin typing the name and a list will populate.

Select the person you want to solicit feedback from. Once a name is chosen, the Select box will be highlighted and you will move to the
next step.
Confirm: The last step will show the name of the person you've
selected. Click Send to confirm. Note: The Leader ratings will not

be visible to others during this step and you will not be able to work on the form until the Feedback Provider sends it back. You may also
find the form in the En Route folder, click on it, and click Recall to
recall the form back to your Inbox.
Note: When you click on the Send to [Team Member] button you will
receive a pop up window and will need to click Send to Team Member one
more time to officially submit the form. If the form is sent back to the Team
Member for revisions, the Leader ratings and comments will not be visible.
