Understanding Consumers Views of Cost Sharing, Quality and Network Choice
The objectives are to explore opportunities and challenges among individuals newly enrolled in the California insurance marketplace (Covered CA), as compared with those who have had coverage for at least 5 years, using focus groups and surveys to (1) identify meaningful attributes of healthcare quality from the perspective of newly insured consumers and those with stable employer-related coverage; (2) determine if and how provider choice and potentially costly treatment decisions are influenced by cost-sharing, network design, and related perceptions of quality; (3) identify the quality-related and other resources that consumers trust when seeking information and advice for health plan and provider choice (and why those sources are trusted); and (4) assess differences in consumer perspectives relative to their experience with health insurance and key socio-demographic characteristics.

Variations in Care: Comparing Heart Failure Care Transition Intervention Effects
This mutli-site randomized controlled was designed to: (1) compare the effect of implementing two separate care transition interventions with concurrent controls on variation in readmissions among older patients hospitalized with heart failure at six medical centers; (2) examine the change in variation over time in readmissions and mortality among hospitalized Medicare beneficiaries with heart failure at the six medical centers; and (3) compare the health benefits and costs of the two care transition interventions.

Translating the AHRQ Quality Indicators to the VA
The objectives of this project were to: 1) develop collaborations with key stakeholders who provided clinical and methodological input and guidance on identifying a set of high-priority QIs for use in VA; 2) apply and modify the AHRQ QI algorithms for use on VA data; 3) investigate the construct validity of the QIs as indicators of quality of VA care; 4) to determine rates of the QIs across VA facilities and Veterans Integrated Service Networks (VISNs); and 5) to develop a national quality report and accompanying informational guide that facilities can use to interpret and better understand their QI rates.

Impact of Resident Work Hour Rules on Errors and Quality
The major goals of this project were to compare changes in the rates of mortality and failure-to-rescue (death after complications) in teaching hospitals and non-teaching hospitals before and after implementation of the ACGME work hour rules. The secondary goals were to examine differences in AHRQ Patient Safety Indicators (PSIs) before and after the rule change and to study how length of stay (LOS), the probability of a prolonged length of stay, and conditional length of stay (LOS once a stay is prolonged) changed in teaching vs. non-teaching hospitals.

Identifying Patient Safety Indicators from Administrative Data
The objectives of this project were to: 1) apply and test AHRQ’s Patient Safety Indicators as a screening tool for potential patient safety events in the VA; 2) validate the PSIs with chart-abstracted data from the National VA Surgical Quality Improvement Program (NSQIP); 3) determine occurrence rates of PSI events across VA facilities and VISNs; 4) identify patient-level, facility-level, and systems-level factors that explain variation in rates of PSI events across VA facilities and VISNs; 5) compare the occurrence rates of PSI events in VA and non-VA hospitals for benchmarking purposes; and 6) explore whether those facilities with lower PSI rates employ specific practices that can serve as "best practice" models for other facilities

Information about Quality in a Randomized Evaluation
The objectives of this project are to: 1) identify factors associated with consumers' use of employer-disseminated information about health plan and medical group performance; 2) determine whether and how consumers use such information during open enrollment; 3) use qualitative methods to explore why consumers consistently request quality information, but then fail to use it; and why they claim to value quality, but then base their decisions largely on price; 4) design and implement an intensive program for disseminating quality information to receptive or "vulnerable" subpopulations; and 5) evaluate the impact of this program by randomizing members of a large purchasing organization to receive existing quality information with or without this supplemental program.

The Outcomes of Hospital Outcome Studies
The objectives of this project were to: 1) evaluate the impact of publicly released hospital outcomes data on AMI, diskectomies, and deliveries in CA, and data on CABG and PTCA in NY, on hospital volumes and referral patterns; 2) evaluate whether this impact varies by age, gender, race/ethnicity, insurance source, diagnostic category, urgency of admission, or market characteristics; 3) explore how hospitals in CA and NY have used outcomes data, and to solicit feedback concerning the optimal content and format of outcomes reports; 4) explore how other interest groups, such as consumer organizations and purchasers, have used outcomes data, and to solicit similar feedback; and (5) to apply these results in developing a new generation of hospital outcome studies that will be more responsive to the needs of consumers, providers, and purchasers.
