PROVIDER FOCUSMARCH 2014ICD-10 Physician UpdateAs we approach the October 1, 2014 ICD-10 implementation date, plans for preparing our staff and providers are in high gear. Almost everyone in all areas of UnityPoint Health will receive some level of ICD-10 education in the coming months. Precyse’s ICD-10 solution has been secured primarily for our employed providers to ensure the content provided to this group is relevant and valuable. The Precyse Physician Documenter Bundle provides a full suite of courses to fully prepare a physician for the ICD-10 transition. These courses put significant focus on teaching the increased specificity required in documentation, how this specificity is to be included in the medical record, and a review of each major system and disease category and their required documentation needs. The Precyse Physician Coder Bundle offers comprehensive ICD-10 coding education to manage the new coding system, the new rules and changes, and how to drive optimal code selection. Both course bundles will be assigned as appropriate. Additional training content will be provided to non-employed physicians through PowerPoint presentations and tip sheets. Our non-employed physician partners will have the ability to purchase access to the same Precyse content. Both employed and non-employed providers will be able to access the content available through 3M free of charge. The full 3M course lists are available on the ICD-10 intranet site and the videos will be available in the near future.Documentation to support ICM-10For inclusion in the ICD_10 Physician UpdateGeneral documentation TipsUse adjectivesAcute ChronicAcute on ChronicMildModerate SeverePersistentIndicate cause and effectUse “due to” or “secondary toBe specific about the aspects of a diseaseUse the most current terminologyPrecisely specify the anatomic siteUse exact dates when appropriateInclude Present on Admission if not obviousRecord all of the conditions that affected the patients current episode of care by any of the following:Clinical evaluationTherapeutic treatmentConsultationsSpecify conditions that were resolved/resolvingSpecify conditions that will monitored after D/CEpic UpdateASAP (ER) Order Association:Starting Tuesday March 18th any order placed for an outpatient procedure will need to have a diagnosis associated to it. Currently there are more than 25,000 registered users. Continued patient adoption of MyUnityPoint is important as UnityPoint Health – Des Moines prepares to meet its meaningful use requirements.MyUnityPoint is going through a change, behind the scenes. UnityPoint Health has switched to Epic’s MyChart patient portal product from Jardogs’ FollowMyHealth (FMH). As a result, there are now two different portals in the Des Moines region until May 2015 – one for hospital records and one for UnityPoint Clinic records. From the patient’s perspective, the MyUnityPoint name will remain consistent. Steps are being taken to minimize confusion and help patients easily connect to their information.Hospital patients are notified of the MyUnityPoint availability on their After Visit Summary (AVS). The instructions walk them through setting up an account and creating a user ID. Clinic patients will continue to be educated at the clinic and through direct marketing.UnityPoint Clinic locations will switch to MyChart as they go live with Epic. Clinics in the Des Moines region go live with Epic in May 2015. Patients should still be encouraged to register for MyUnityPoint by visiting chart.. There, they will have access to both portals. For questions and feedback about MyUnityPoint, please contact 877-224-4430 or myunitypointsupport@.ACTION ITEMS:Allowing patient access to an online health portal and having patients actually use the portal is a Meaningful Use requirement. Meaningful Use measurements begin in April 2014 for UnityPoint Health – Des Moines. Please share, with your staff, the importance of communicating with hospital patients about the opportunity to access their records once they return home.Show staff the location of the MyUnityPoint information on the AVS.
