CONSENT AND RELEASE FORM

Medical and Recorded Images

I agree to relinquish to United Way Fox Cities, Inc. (hereafter UWFC), all rights, title, and interest that I may have in the finished recording, reproduction, and copies of the original video and/or audio recording(s); or, in the prints, negatives, reproductions, and copies of the original photographs(s) and negative(s).

I also irrevocably grant to UWFC permission to put any of all recording and photographs to any legitimate uses UWFC may deem proper including the right to use for purposes of advertising and trade.

I do hereby give my consent allowing my child to participate in United Way Fox Cities Youth Board activities. Furthermore, the UWFC staff supervising these activities may act in my stead, including authorizing medical treatment for my child. I will not hold UWFC or its staff fiscally or legally responsible for injuries or damages incurred during these activities and waive all rights to legal redress.

What it takes to be a Youth Board Member

Youth Board members are high school students who are interested in becoming more involved in their school and community. Through participation on the board students learn about many different organizations that exist through the help of the United Way. Board members bring back to their school peers and community organizations information they learn on the various volunteer and grant opportunities for youth.

Responsibilities and Expectations of a Youth Board Member

I agree to…

YES NO Attend meetings on the 1st and 3rd Mondays of each month from 6:30-7:30 PM.

YES NO Attend and actively participate in 75% of Youth Board meetings.

YES NO Email, text, Facebook, or call a Youth Board Co-Leader or Advisor myself if I must miss a meeting for an excused reason such as an illness, extra-curricular activity, or family function.

YES NO Attend and actively participate in at least 2 community service projects per semester.

YES NO Attend special events such as the Teen Symposium event and other events throughout the year.

YES NO Serve in a liaison role to their respective schools.

YES NO Announce availability of funds to student organizations in school and distribute grant applications to interested groups; participate in a decision-making process to fund grant requests.

YES NO Help with recruitment of new Youth Board members.

YES NO Do my best to fulfill the responsibilities and expectations of an active Youth Board Member.
