Per Illinois law: All students entering 6th grade must show proof of having a physical examination within one calendar year prior to the date of entry. The examination must be reported on the NEW State of Illinois Certificate of Child Health Examination form (dated 11/2015 on bottom left corner) & include a complete immunization record and health history (to be completed & signed by the parent & verified by the health care provider). This physical exam must be turned in prior to the 1st day of school.

*Please note -A Sports Physical reported on an IESA form is NOT acceptable as a school physical examination.

Per Illinois law: All children in 6th grade shall present proof of a dental exam prior to MAY 15th of the school year.

*Section 27-8.1 of the Illinois School Code states: "If a child does not comply ...... then the LOCAL SCHOOL AUTHORITY SHALL EXCLUDE THAT CHILD FROM SCHOOL until such time as the child presents proof of having had either the health examination or those required immunizations which are medically possible to receive immediately."

*Parents or guardians who object to health examinations or immunizations on religious grounds must present a Certificate of Religious Exemption signed by a health care provider. If a child cannot be fully immunized because of health reasons (medical contraindication), the physician must provide the school with a signed statement.

*In the event of one diagnosed case of a communicable disease such as measles or polio, any student who is not protected by immunization shall be immediately excluded from school. This exclusion shall be in effect for the required number of days (determined by the health department) following the last reported case or until the student can show proof of immunity.
