Real World, Real Choices
Solution and Rationale
The decision for which type of catheter to deliver the chemotherapy for this patient requires additional information. If he is going to be at home and requires chemotherapy for a moderate length of time a peripherally inserted central catheter (PICC) line is not the best choice. The rationale for not using the PICC is that this type of catheter is typically used in the hospital setting as it is vulnerable to infection and is not suited for moderate- or long-term use. There is not enough information to make a decision in favor of the implanted port type of central catheter. Therefore, the choice of a tunneled central line (e.g., Hickman or Broviac) would make a good selection, as these types of catheters can remain in place for months at a time, as long as the catheter site remains healthy. Assuming that the chemotherapy is going to need to be administered over a relatively lengthy period of time the tunneled catheter is a good choice.
