Text for Recruiters to explain survey & consent page

When local recruiters approach a potential participant, begin by introducing yourself to the person and telling them about the survey using the script provided below. If a person agrees to participate, make sure you ask them 2 questions: 1) Have they already taken the survey this year? & 2) Are they 18 or older? If they have already taken the survey or are not yet 18, they should not take the survey. If they are willing to take the survey and meet the requirements, use the text at the bottom to describe the consent page. It is absolutely essential that the participant is described the consent form in case they don’t take the time to read it.

Text for introducing yourself:

Hi, I’m __________(e.g., John) from ____________(e.g., San Juan County Partnership) here in ____________(e.g., Farmington). We’re conducting a survey to find out what folks know about prevention efforts going on in the community. It takes about 10 minutes to complete and as an incentive, we’re giving out _________ (e.g., $1.00 off coupons on your grocery bill). Would you be interested in helping us today?

If YES…

Have you already taken this survey?

If YES ---“That’s great! Thanks so much for doing that. But we’re asking that

people take the survey only once.”

If NO --- “Great. Well, let me tell you a little bit about it. It takes about 10 minutes to complete and it asks questions about what’s happening in our community, meaning __________ (e.g., Farmington) related to drinking and drinking and driving. Are you still interested in participating?

If NO ---- Thank you for your time.

If YES --- “And are you at least 18 years old?” (Ask if person looks younger than 50)

If NO ---- “I’m sorry, but you must be at least 18 or older to take the

survey. Thanks!”

IF 18 OR OLDER….

Text for explaining the survey & consent page:

“Great! Here’s the survey. (Provide a survey on a clip board and provide a pencil or pen.) As I mentioned before, the survey is about substance abuse prevention in our community. This first page explains to you your rights when taking the survey. Most importantly you should know that this survey is completely voluntary, you may skip any questions you don’t want to answer, and your responses are anonymous, so do not put your name anywhere on the survey. If you have questions along the way, please feel free to ask me and I’ll do my best to answer them. Finally, if you find as you start to the take the survey, that you’ve already taken it recently, please let me know.

As you answer the questions, please think about them in the context of our community, ________ (e.g, Farmington). Please just provide one answer per question, unless it says you can answer all that apply in which case you can provide more than one response.”
