FY19 MHVI QI LC Team Charter - Family Retention and …
Collaborative Problem Statement: In order for home visiting programs to positively impact family outcomes, engagement of families for the duration specified by the model is critical. Currently, a higher percentage of families than preferred are discontinuing home visiting services prior to completion. While Local Implementing Agency (LIA) QI projects focused on family retention and engagement in the past have yielded successful improvements in this area, there is room for continued improvement. Collaborative Aim Statement: By September 1, 2019, the MHVI Retention and Engagement QI Learning Collaborative aims to have no more than 3% of families who stop services before completion._____________________________________________________________________________________Team Name: Choose an item.PDSA Cycle # (Please indicate which PDSA cycle number this charter reflects within this year’s Learning Collaborative work, e.g. FY 18 LC cycle #1, 2, or 3, etc.): Choose an item.
