To apply for a DBS Disclosure you need to produce original documents so that your identity can be verified; one of these must verify your current address and all documents must be in your current name.

The checklist below shows the valid identity documents that you must bring to use when completing your DBS application form. The Recruiting Manager will initial the checklist to confirm which documents have been seen, and will also take copies.

We require THREE documents. At least one document MUST be from Group 1, the other two should be from Group 1 or Group 2 (a) or (b).

If you are UNABLE to produce any document from Group 1 we require THREE documents from Group 2, comprising of one document from Group 2a and two further documents from Group 2a or 2b. The Council is then required to undertake an external ID validation service check to establish the applicant’s name and living history footprint. Information on this check is available at .uk/dbs. If you fail this check, it may be necessary to go for fingerprinting; full details will be confirmed with you, should this be required.

GUIDANCE FOR APPLICANTS

When completing the DBS application form you will need to provide your passport number, driving licence number and five years of continuous address history. These must be provided even if you are not going to use one of these documents as identity.

The following bullet points provides information on how you can obtain the required documents

- If you cannot find your driving licence you must call the DVLA on 0300 790 6801 to report it lost then either order a replacement for £20 or mark Yes on the DBS form and enclose a signed note saying that it has been lost for X amount of years and the DVLA has been informed.

- You can obtain a copy of your birth certificate, marriage certificate, civil partnership certificate and adoption certificate from the General Registry Office:



o In order to obtain your certificate you will need to provide the GRO index number, this can obtained from the following website

o The GRO number is made up of:

▪ Year (of the certificate)

▪ Volume number

▪ Page number

▪ District in which the event was registered

- Bank Statements: If you usually bank online you can request a printed copy to be posted to your home address by the bank either through your online banking system or in the branch itself. Please note that we CANNOT accept bank statements that have been printed at home; statements must be sent to you from the bank.

- If you live within the Brighton & Hove City Council area, you can request a copy of your Council Tax bill by either calling 01273 291291 or emailing council.tax@.uk and providing your name and post code.

- For all other documents listed in Group 2b you should be able to request a copy from the issuing organisation to be sent to your home address
