Player Contract and Parent Code of Conduct

Please read the following information and check the box to indicate your agreement:

The undersigned hereby give permission for my child, noted as "Player," to participate in the

Northern Kentucky Fast Pitch League (NKFPL). It is understood that the

NKFPL is a non-profit, volunteer organization, which sponsors the above-noted activity as a

community service, and is not responsible for expenses or damages resulting from an injury to

players and/or spectators which are sustained in conjunction with, or incidental to, the above noted

NKFPL sanctioned activity.

Accordingly, the undersigned HEREBY EXPRESSLY AGREES to waive any and all claims against,

and hold exempt from liability, the NKFPL, its officers, directors, coaches, volunteers, and/or any

other person that may be affiliated with the NKFPL, for any injuries sustained by my child, from

whatever cause, while attending, participating in, or traveling to or from any NKFPL activity or

event.

IT IS FURTHER AGREED that undersigned HEREBY EXPRESSLY AGREES to waive any and all

claims against, and hold exempt from liability any and all claims against the Division of Amateur

Athletics of the Department of Recreation and Parks (Rec & Parks) of Campbell County, its

supervisors, managers, volunteers, and/or any person that may be affiliated with Rec and Parks, for any

injuries sustained by my child, from whatever cause, while attending, participating in, or traveling to or

from any NKFPL activity or event.

To the best of my knowledge, my child is physically fit and able to play softball, and I hereby agree to

furnish the NKFPL a doctor's statement to that effect when requested.

[ ] CODE OF CONDUCT:

Understanding that parents/guardians are the singular most important role model for their

children, I hereby agree to:

1. At all times, I agree to maintain and display self-control and patience while attending practices,

games or tournaments in which my child is participating;

2. To provide positive encouragement as a spectator since I believe it is the best way to support

the team and my child's athletic endeavors;

3. To teach my child to play by the rules, respect game, league officials, umpires, and respect

others, because that is as important as learning the skills necessary to successfully play a

sport;

4.I recognize that the head coach can best represent the team when dealing with any disputes

with the umpire(s). I hereby agree that the head coach, or someone designated by the head

coach in his or her absence, will be solely responsible for handling all pregame, league and

tournament games, and all post-game disputes with the umpirets) and/or league officials

and/or opposing coaches.

5.1 understand that participation in this league is voluntary and privileged. Conduct and actions at

league activities that are not within the best interest of the league may result in my removal

from such activity, and ultimately jeopardize my and my daughter's participation in league

activities.

6.I also agree that if as a result of my actions, local law enforcement is called to respond to

a NKFPL game, I understand that a disciplinary action, including, but not limited to,

suspension, and/or expulsion from all future games may levied against me by the Board of

Directors of the NKFPL.

7.I have read the Code of Conduct, and I have explained and discussed this with my child. I

acknowledge that if I violate any of the above-referenced requirements, the Board of

Directors of the NKFPL may initiate a disciplinary action against me, including, but not

limited to, suspension, and/or expulsion from any and all future NKFPL sanctioned games.

8.Finally, I understand that the NKFPL is about the players developing a sense of teamwork,

sportsmanship and self-worth, and I fully support these ideals.
