If no response in 10 minutes, may repeat bolus of ketamine 10 mg. After bolus, increase rate of ketamine infusion by 5 mg/hr (not to exceed 1 mg/kg/hr unless approved by APMS physician). May be repeated as needed, but no more frequently than every 60 minutes. Document therapy and outcome on Titrating Analgesia flow sheet. ................
