Full payment for your birthday party is due one week before your birthday party is held.)

In accepting this contract, you acknowledge the following:

1. Other parties may precede or follow yours, so the 1 1/2 hour total time limit must be adhered to (1 hour gymnastics, 1/2 hour party time. Please do not arrive more than 15 minutes before your scheduled time.) Also, please have all children wear sneakers to use our rock climbing wall.

2. Parkettes provides 1 hour of supervised gymnastics activities, party invitations and bags, tables, chairs, and plastic table covering. It is your responsibility to provide cake and drinks (refrigeration is available), plates, forks, spoons, napkins, etc

3. The deposit for your party is your confirmation. Your time slot cannot be held without the deposit. The Monday before you party is scheduled, please call in a head count, as well as your final balance (the balance is your party fee minus the deposit.) If your amount of children exceeds the number on your contract, you will be charged the next fee up, ($230.00, $295.00) and no coach will be available, unless you call four (4) days ahead, so please let us know immediately if your total number of children changes.

4. IT IS THE HOST FAMILY'S RESPONSIBILITY TO DISTRIBUTE, COLLECT THE RELEASE FORMS LOCATED ON THE BACK OF YOUR PARKETTE INVITATIONS, AND TURN THEM INTO YOUR BIRTHDAY PARTY INSTRUCTOR BEFORE YOUR BIRTHDAY PARTY BEGINS. ANY CHILD WITHOUT A RELEASE FORM SIGNED WILL NOT BE ALLOWED TO PARTICIAPTE.

5. If the gym should be closed due to inclement weather, or because of a meet in the gym, every effort will be made to reschedule your party at a time satisfactory to you.

6. For safety reasons: No children are permitted on the floor areas or equipment before or after the 1 hour gymnastics time. Siblings that are not participating need not be included in the guest count, and parents must remain off the floor areas during the gymnastics portion, except to take pictures. Any child under 3 years old, MUST have a parent with them at all times on the gym floor.

7. RELEASE AGREEMENT: Acceptance of this contract for a Birthday Party is without assumption of responsibility of any kind by the Parkettes program for injuries or losses which may be suffered or sustained in connection with activities during the period of the party. Birthday party participants are expected to carry their own accident and/or medical insurance. Coaches and Instructors of the Parkette program are safety conscious and follow appropriate safety procedures. In the event of injury or illness every effort will be made to contact the parents or guardian. If necessary, I authorize the Parkettes to administer first aid and/or medical treatment.
