DAY CARE PROVISIONS AND PARENT-PROVIDER AGREEMENT

Fees and Hours of Child Care: The contracted hours of care for

___________________________ , in the home of (Your Name Here) are between the hours of _____ (am/pm) and _____ (am/pm) for days per week, on the following days of the week:___________________________________________.

Fees: The basic charge shall be $1.75 per hour. You may set the date you wish to pay for your child’s care, however, payment must be made at least every two weeks. A penalty charge of $20.00 will be assessed for each check returned. In addition, the parent will pay for all expenses to the provider's account resulting from the returned check. In the event of a second returned check, all childcare must be paid in advance or the provider may choose to terminate service. The provider reserves the right to adjust the fees at any time with at least two (2) weeks written notice.

Late payment: A fee of $10.00 will be assessed if the fee has not been paid by the end of day immediately following the payment due date. If payment is not received the next business day, the provider may terminate service without notice and the deposit and any fees are non-refundable. Remember, you set the date in which you will pay for your child’s care.

Leave or Vacation

Paid Holidays: (YOUR NAME HERE) Daycare will be closed on the dates indicated on the calendar. Payment for absences is optional, however, if you get paid, I would appreciate you extending the courtesy to me.

Vacation Leave: (YOUR NAME HERE) Daycare will be closed in July, and will also be closed the week between Christmas and New Years. A calendar is attached to this Agreement indicating the scheduled days that (YOUR NAME HERE) Daycare will be closed. It is the parents' responsibility to obtain substitute care during such times. Payment is not required for times when the provider is on vacation.

Sick/Personal Leave: Although the provider will make every attempt to be available each day, there will be occasions when she or another family member is ill and she is unable to provide service. Parents will be notified as soon as possible of any personal or family illness, funeral, or emergency. The provider will make an attempt to find a substitute for these instances, however, in the event she is unsuccessful in finding a substitute, it will be the parent’s responsibility to obtain substitute care.

Child's Absences: The parent should inform the provider as soon as possible if the child is to be absent due to late arrival, early departure, illness, or vacation. Children who will be absent due to illness or vacation for a period longer than three days will be required to pay one-half the regular cost of your child’s care.

Illness: Under no circumstances will parents bring a sick child to the provider's home. A sick child should be allowed to recover fully after an illness so that other children in the group do not risk exposure and so that the child is able to fully participate in childcare activities. If you are unable to remain at home with your sick child it will necessary for you to make substitute child care arrangements at your own expense.

Supplies: The parents will provide formula, disposable diapers, wipes, and diaper rash ointment. If the child is toilet training, parents will provide training pants or pull-ups and a change of clothing.

Your child is welcome to bring toys and games to the day care that he/she may wish to share with the other children. Note: Please write your child's name on all of his/her belongings. The provider is not responsible for these items.

Nourishment:

The provider will serve nutritious meals and snacks. Meals will be served at the following times (approximate):

Breakfast 7:30 am

Lunch 11:30 am

Snack 3:30 pm

Please do not send money, gum, candy, or any other food with your child unless

prior agreement has been made.

If baby food and bottles are being used, parents are responsible for supplying these. If a child requires a temporary special diet, the parent is responsible for bringing these food items. The meals should come commercially prepared, ready to heat and serve.

Pre-School Learning Program: Parents have the option of enrolling their child in the Pre-School Learning Program, which is a preschool curriculum and craft program. Pre-School Learning Program is a fun and educational activities program for 3 to 5 year old children. Children will participate in the program three days per week. The fee for this program is $10 per month and is payable with your first payment each month. The cost of this program will decrease depending on how many children are enrolled; however, it will never cost more than $10 per month. Children who are not participating in the program will do other sit down activities such as coloring, puzzles, etc.

I encourage you to participate in the program as it provides hands-on learning, builds self-esteem and is very inexpensive.

Field Trips: Occasionally, the provider will take the children on neighborhood walks, field trips, or other out-of-house activities that involve transportation of children by foot, in strollers, or in vehicles owned by the provider.

The parents authorize that the child may participate in planned field trips or activities. Advance notice of all trips will be given except in cases of emergency not involving your child. The parents agree to pay for any admission charge and other expense involved in the field trip, if required.

The provider will observe all laws and regulations set forth by the state motor vehicle department. Each child will be securely belted or placed in a regulation car seat. In the unlikely event of an accident, the parent will be notified as soon as possible.

Special Days: The provider will observe special days with activities (e.g. birthdays, Halloween, Valentine's Day, etc.). You are welcome to bring decorations, games, or treats to help celebrate as long as there is enough for everyone. This can be discussed with the provider in advance. If you have any objection to your child participating in the any of these holidays, please discuss this with the provider.

Nap and Rest Time: All children will have a nap or rest time each day. These times will normally be scheduled after lunch.

Agreement

I/we have read this contract and will comply to all provisions contained herein, and shall at this time enter into agreement with (YOUR NAME HERE) for the care of my/our child, ________________________ , with the understanding that we shall work together on behalf of the child.

This agreement is in effect until a change is mutually agreed upon in writing or upon termination of care.

This agreement is subject to review and renewal on. Any changes made by the provider to the terms of the agreement must be made on the renewal date unless mutually agreed to beforehand by the provider and parents who are parties to this agreement. Otherwise, this agreement will remain in effect until the renewal date or upon termination of care as set forth herein.
