The following procedures MUST be followed in order for the request to be reviewed by the Section Commissioner
Note: CIF and the Los Angeles City Section eligibility rules may only be waived because of an unforeseeable, unavoidable, and uncorrectable act, condition or event which causes the imposition of a severe and non-athletic burden upon the student or his/her family (hardship.) Parties filing a hardship waiver request MUST provide proper and complete written evidence (documentation) in support of the request, and must do the following:
1. Discuss the waiver request with current school’s athletic personnel to determine what documents are required.
2. Attach a brief statement to this form regarding the nature of the problem and the reason for requesting a waiver for eligibility. The reason must meet the criteria stated above.
NOTE: The following documents must be attached to this form, based on the rule for which the waiver is requested.
A. A written statement from the parent/guardian identifying the reason for the request of the CIF bylaw(s).

B. Official school transcript, indicating academic progress throughout high school. (Rules 203, 204 and 205)
C. Attendance records (apportionment) for semester(s) in question. (203, 204 and 205)
D. Computerized Student Information (ID01 or similar document provided by the school). (203, 204 and 205)
E. Copy of latest report card (if not included on transcript.) (203, 204 and 205)
F. Signed and dated written statements from doctor or other healthcare professionals and dated medical records. (Needed if the waiver is for medical reasons)
G. Courses completed and courses pending toward meeting graduation requirements. (203, 204 and 205)
H. Any other records deemed appropriate to prove the hardship to justify the waiver request.

FILING A REQUEST FOR WAIVER
Student must meet all CIF athletic eligibility requirements except the waiver requested to qualify to submit a hardship waiver application. In order to file a request for a waiver of CIF Los Angeles City Section, please follow the procedures as indicated on the reverse side of this document. After speaking with school athletic personnel, if it is determined that a waiver request is to be filed with the Section Commissioner, once all pertinent paperwork is submitted to the Section Office a decision will be rendered within 20 days. It is the duty and responsibility of those parties making the request for a waiver of a rule to provide all necessary documents, should more documentation be required the 20 day timeline will restart from the date received. Required information includes but not limited to:
• Transcripts
• Attendance records
• Cumulative records identifying completed graduation requirements and needed course to meet graduation requirements.
• Documentation that is specific to support the claim of hardship. Decisions are based on a preponderance of information and documentation provided.
• IEP & EL in and of themselves do not qualify for hardship waivers.
• Information that would connect the hardship to the targeted semester requesting exception waiver to apply to.
Parent and/or school personnel will be notified of request for additional documentation. Please note that cases will be handled on a priority basis for those requests that are most applicable to the upcoming season of sport.
Upon receipt of paperwork in the Section Office, the Commissioner will confirm receiving the waiver request and documents with the student’s parent(s) and/or school personnel. In the event it is necessary to obtain further clarification of facts, a meeting will be scheduled between the Commissioner and parent(s)/guardian/caregiver to provide opportunity to provide further clarification of hardship. Although students are not required to attend, students are welcome to attend, based on the permission of the parent/guardian.
All meetings are held at the CIF Los Angeles City Section Office, unless otherwise notified. For the 2020-2021 until further notice meetings will be conducted via Zoom. See current information at cif- under “Athletic Directors/Transfers/Transfer Forms” Hardship Waiver Request.
School administration and parents will both be notified of the decision in writing.
