Dear Teen Leadership Camp Applicant,

The 2nd Annual Teen Leadership Camp will be held from June 14-19, 2009 in Lebanon, PA at Kenbrook Camp and Conference Center.

On this website you will find a number of forms that are required to be filled out during the registration process. Below is a list of these forms with information about when in the registration process you should have each form returned. Forms can be mailed or faxed to the address at the end of this letter.

This Teen Leadership Camp is not for everyone. Selection is stringent, and participants will be chosen based on their maturity level and sincere interest in becoming leaders. Unlike the other summer camps for military children, Teen Leadership Camp is an educational experience and not just a recreational event. If this type of experience sounds like it interests you then I encourage you to return the first three forms ASAP.

RETURN THESE 3 FORMS PRIOR TO MAY 15

Registration Form – This is the first of two forms that need filled out and

returned prior to May 15. This form will be reviewed when choosing participants.

Media Consent Form – This is the second of two forms that need filled out and

returned prior to May 15. You will not be accepted into this camp experience

without this form on file.

Short Leadership Essay – The third required form to be considered as an

applicant for the 2009 Teen Leadership Camp. Please write a 300-500 word essay that defines your view of leadership and why you want to learn more about leadership. You should also explain why you want to be considered to attend this camp. Essays should be type written. Include this essay, along with the above two forms, in your initial application package.

RETURN THESE FORM AFTER ACCEPTANCE TO THE CAMP

Health History Form – This 2-page form must be filled out by your parents/guardian and signed by your doctor. Do not pay for a physical until you

have been notified of your acceptance. It is advised that you make a Dr.

appointment to get a camp physical in early June so you don’t have to scamper to get an appointment if/when you get accepted. This way, if you do not get accepted you would not have spent money without cause. It is acceptable to use an alternative physical form such as one from a school sports physical if it is signed by a physician and dated within one year from June 14, 2009.

Medicine Distribution Form – This form will be submitted in late May or

early June. All campers will be required to fill out and return this form even if

you will not be taking prescriptions or over the counter medicine. A blank form is

indication to us that you are not on medication.

Let me take a moment to explain what Teen Leadership Camp will involve. The PA Operation Military Kids (OMK) is funding this camp. You will learn more about OMK while you are at camp this summer.

There will be approximately 20 teens admitted into this camp. Participants will be chosen based on their desire to learn and grow in their knowledge of leadership. Professional leadership mentors that are excited about working with our National Guard teens will staff the camp.

The 6 day/5 night experience will be a three part experience revolving around traditional camp activities such as swimming, sports, fishing and boating. In addition, fun interactive leadership courses which are designed to hone your leadership skills and to peak your interest in continuing your mission to become leaders will be offered. Lastly, you will have field trips to near-by Fort Indiantown Gap, where you will be actively involved in the development and filming of a video which will explain to the public what it is like to be a military kid.

Overall, I believe this experience will be a fun and productive opportunity for all participants! I know I am looking forward to meeting our next generation of leaders.

I plan to accept all applicants by May 20th. You will receive an email with acceptance/waitlist information and further instruction on arrival and departure times. Good luck to you.
