LDL LED Manufacturers Checklist

The LDL LED List is an interim qualifying list while your product is being reviewed by ENERGY STAR or the DesignLights® Consortium Products will be removed 12 months after they are posted to the list. If you want to resubmit the same product after its initial 12 months on the List, it MUST have a new LM79 test.

PLEASE READ THESE INSTRUCTIONS CAREFULLY!!!

Submittal emails with incomplete documents will be deleted, and you will be asked to re-submit all documentation.

Attach the following documents as a PDF file and email to: ledlist@. Submit only ONE PRODUCT per email. Put mfg. name and model # in the email’s SUBJECT line; e.g. Lucky Loo LED, #LL-P38/827/41K-80

1. LDL LED Manufacturers Checklist (Scanned copies will not be accepted)

2. LM-79 Report (Testing lab must be approved by Lighting Facts® approvedlabs)

3. Product warranty

4. Product spec sheet

5. Proof of safety certification such as UL, ETL or CSA-US (must be NRTL)

6. Product sample in production packaging (for integral LED lamps only, and within 30 days of submittal)

Please allow 3-4 weeks for processing.

Common reasons why submittals are rejected:

1. The most current submittal documents and checklist (found on our website) were not used.

2. The model or SKU number on the checklist did not match the model or SKU number on the

LM-79 report, UL certification document or the product spec sheet.

3. There were blanks in the checklist.

4. The test report was not from a LM79-certified testing lab.

5. The test report was not final.

6. Documents were missing (especially warranty and UL report).

7. The email contained documentation for more than one product.

8. Values on the checklist did not match values on the LM-79 report or the spec sheet.

9. Documents that were not required were attached.

10. Values in the checklist were hand-written or scanned.
