Upon review and approval of your request, the funds will be sent to you according to your predetermined method of payment. Either a check will be mailed to you or a direct deposit will be sent to your bank account. This disbursement will be made within five (5) business days of our receipt of this request. Please ensure all fields are completed prior to submitting the request. Incomplete forms could cause your request to be delayed or denied.

IMPORTANT: Please do not reuse this form. A new form and a new signature are required for each draw request. Copies of previously submitted forms will be denied.
