|1.2.1 |Please explain briefly your organization’s history, how and when it was formed. |

| |Miss Naseema Hurzuk, President, was a normal healthy girl taking part in sports, cultural programs, and doing well in studies. At the|

| |age of 16 she became a paraplegic due to spinal injury. After initial trauma she decided to overcome her disability and to help |

| |herself and others. She completed her graduation and got a job in Central Excise Dept. Due to her integrity and hard work, she rose |

| |to the position of Deputy Office Superintendent. During her employment she used to help physically challenged students with school |

| |/college fees, getting medical treatment etc. She wanted to help the physically challenged in better and more organized manner. |

| |Accordingly, with the help of a few like-minded friends she formed the organization ‘Helpers of the Handicapped, Kolhapur’ in 1984. |

| |The organization has been consistently progressing since then. |

| |The organization has been consistently helping the physically challenged with medical assistance (including surgeries), aids and |

| |appliances, educational aid, vocational training, etc. The following major projects have been undertaken. |

| | |

| |Gharonda – Hostel-cum-Rehabilitation Centre (HCRC) – |

| |Operational since June 1996 |

| |We realised early that the physically challenged from the rural areas find it extremely difficult to go to schools / college mainly |

| |due to inadequate and unsuitable public transport. We therefore planned for a Hostel, which would be suitable for them. We received |

| |a plot of land admeasuring about two acres at Uchgaon, Kolhapur, from Government of Maharashtra. With substantial grant (Rupees 56 |

| |Lakhs+) from Manos Unidas of Spain we constructed Gharonda our Hostel-cum-Rehabilitation Centre. At this moment there are about 125 |

| |individuals (most of them physically challenged) living here. Out of these there are 72 students and remaining are vocational |

| |trainees, and staff. The custom-built hostel and our own transport facilities suited for physically challenged students makes it |

| |possible for them to go to schools / colleges and become educated, self-reliant and productive members of the society. |

| | |

| |Vocational Training Centre (VTC) |

| |Operational since 1993 |

| |There are a number of physically challenged who are school dropouts or do not have the mental capacity to pursue academic education. |

| |These are given vocational training in our Vocational Training Centre. We have leased out a building from Kolhapur Municipal |

| |Corporation for this purpose. In this VTC we manufacture wheel chairs, callipers, crutches, portable commodes, as well as office and |

| |school furniture. All staff in VTC is physically challenged. This Centre gives them training in metal fabrication, welding, carpentry|

| |and prostheses making. After a stint in the Centre the trainees can go out and find gainful employment or set up their own small |

| |units. |

| | |

| |Samarth Vidhya Mandir – Primary School Operational since June 2001 |

| |This is a primary school situated adjacent to our HCRC at Uchgaon, Kolhapur. It is open to both physically challenged as well as |

| |normal students. At the moment we have about 240 students studying from Class I to Class V. |

| | |

| |Handihelp Gas Agency – Operational since May 1993 |

| |To provide employment to the physically challenged and to augment the institution’s income, a cooking gas distribution agency was|

| |taken from Indian Oil. Many of the employees here are physically challenged. This unit gives them a platform to learn clerical / |

| |office work, cash management and public relations skills. |

| | |

| |Handihelp Nagari Pat Sanstha (Co-operative Credit Society) |

| |Operational since 1994 |

| |To inculcate saving habit among earning physically challenged, and to generate some i income for the institution the Co-operative|

| |Credit Society was established about 10 years ago. This is managed by physically challenged people and has met its objectives |

| |satisfactorily. |

| | |

| |Lajawab Mobile Canteen |

| |To augment our resources as well as to provide employment and training to the physically challenged we started this mobile |

| |canteen in August 2001. Shri Dawood Abid of Pune donated the canteen bus. The bus is parked at public places and at village fairs|

| |and community events. It is fully managed by physically challenged people. |

| | |

| |Solace i.e. Dilasa (Centre for Mentally Challenged and Severely disabled) |

| |In January 2001, we leased out a unit in Sai Baba Seva Dham at Kanhe Phata, Dist. Pune for mentally challenged and severely |

| |disabled. At this moment there are 40 individuals. The students are attending a special school. Others are given training in |

| |managing themselves and certain activities that can sustain them to live independently. |

| | |

| |Swapna Nagari – Dreamland, at Sindhudurga District, Maharashtra |

| |There are about 3,500 physically challenged in the Sindhudurga District without any worthwhile support. We were helping them |

| |externally but always wished to have a base there. Fortunately, Mangaonkar family of the region donated 13 acres of land in April|

| |2001. With generous help (Rs. 50 Lakhs+) from Manos Unidas of Spain we have constructed ground floor of a |

| |Hostel-cum-Rehabilitation Centre. We have received approval for a primary school and Class I has been started since July 2004. |

| |Agricultural activities like farming, planting of fruit bearing trees, cashew processing has been undertaken. A small dairy farm |

| |is operational. A Vocational Training Centre would soon be operational. A home for aged disabled is planned and we are |

| |approaching sponsors for this purpose. |

| | |

| |This project would rehabilitate a number of disabled persons from the backward rural region of Sindhudurg. It would provide |

| |quality education and relevant agro-based vocational training. |

| | |

| |To highlight our activities in various fields, we are attaching a document, ‘Work At A Glance’, at Attachment 2 |

| |working and problems faced by the physically challenged as well as the way they overcome it. Every year we allow around 10 |

| |students of Master of Social Work course to complete their orientation visit and Field Work Placement assignments at our Hostel. |

| |It has been observed by us that rural based physically challenged students find it extremely difficult to commute to their |

| |schools / colleges. This is to due to difficult rural terrain and inadequate and erratic public transport, which does not take |

| |into consideration the difficulties faced by wheel chair bound or those wearing callipers or using crutches. Moreover, the |

| |economic difficulty faced by most of these students’ families restricts the expenditure that a family can spend on a physically |

| |challenged member. All these factors make it difficult for such students to get academic education and hence they remain |

| |illiterate or under educated. |

| | |

| |If we are able to provide them a barrier free custom built accommodation, suitable transport to their schools / colleges, free |

| |boarding, all other facilities conducive to their personal growth, they would be able to acquire academic education. This in |

| |turn would help them to acquire gainful employment. Once employed, they would also be able to support their parents, and other |

| |members of their families. They would be able to find an educated life partner and start a family. Education and gainful |

| |employment of one physically challenged person has a ripple effect on the entire family and the society in general. |

| |The physically challenged would then be fully rehabilitated and integrated in the society as any other normal individual. This |

| |is our Long Term Goal ! |

|2.4 |Project Activities: Please describe in detail the various activities that need to be carried out to successfully complete the |

| |The project is for sponsorship of comprehensive rehabilitation of 72 physically challenged students living in our Gharonda |

| |Hostel at Kolhapur for one year. Since the hostel is already well established for several years, and we have our own buses, no |

| |infrastructure cost is to be incurred. |

| |Description of various awareness methods used (print material, projection slides etc.) Miscellaneous material that needs to be |

| |The organization has printed its own brochure giving brief synopsis of its activities. This brochure is updated periodically. |

| |The latest one was printed on 15 Dec 2004. A copy is enclosed herewith for your perusal. The brochure is printed in English as |

| |well as Marathi languages. |

| |The organization’s activities are widely covered by Marathi press. Newspapers like ‘Sakal’, ‘Pudhari’, ‘Lokmat’, ‘Kolhapur |

| |Times’ etc., give wide coverage to the organization’s activities. |

| | |

| |The students of Gharonda have developed a cultural program called ‘Hum Honge Kamyab’. One of the highlights of this program is |

| |the ‘Wheel Chair Dance’ where a number of students perform dance on their wheel chairs. This has become very popular and several|

| |performances have so far been held in Kolhapur, Mumbai, Pune, Goa, Hyderabad, Ratnagiri, and even Bhubaneshwar, Orissa. This |

| |program serves as a platform to inform the attendees about our activities. |

| | |

| |We hold functions like ‘World Disabled Day’ and distribute awards like ‘Apang Mitra’ (Friend of the Handicapped), and ‘Bharari’ |

| |(Great Flight). These functions also help to propagate our organization’s message to people. |

| | |

| |Miss Naseema Hurzuk wrote a column called ‘Chakachi Khurchi’ (Wheel Chair) in Sunday ‘Sakal’ newspaper. The series of articles |

| |gave wide publicity to our organization’s activities. The articles were later compiled and published in a book form. The book is|

| |now being translated in English, Hindi, Gujarati and Telugu languages. National Association of the Blind, Mumbai have produced |

| |this book in ‘Talking Book’ form for the benefit of the blind and the illiterate. This book though autobiographical in nature, |

| |revolves round the organization and it’s effort for total rehabilitation of the physically challenged. |

| |We are in the process of making a video film depicting our activities. This film is financed by charitable organizations in |

| |Canada. This will help us to reach more people through distribution of CD’s of the film. |

|2.5 |Project Sustainability and Evaluation |

|2.5.1 |How do you propose to measure the effectiveness of the project? Please specify and detail any clear or quantifiable yardsticks. |

| |The effectiveness of the project would be periodically reviewed by looking at the academic, social, and health performance of |

| |the sponsored students. We aim to reduce their disability, and encourage them to excel in their studies and extra-curricular |

| |activities. The results of their unit tests, term examinations would be carefully reviewed. Counseling would be provided as and |

| |when necessary. Any problems in their schools / colleges would be sorted out amicably. |

|2.5.2 |How does this project affect the children involved, their families, and the local community? |

| | |

| |This project would help the students to achieve academic education to the best of their capacities. This in turn would help them|

| |to acquire gainful employment and become useful member of the society. |

| | |

| |Since these children come from backward rural areas their families are low-income labors or small farmers. Since they would have|

| |other children too, it is difficult for their time, energy and meager financial resources on treatment and maintenance of a |

| |physically challenged child. Once our organization takes the responsibility of their lodging, boarding, education, medical |

| |expenses, the family is freed from this responsibility. The family then can devote its time, energy, and financial resources |

| |towards looking after other members of the family and towards their occupation. This helps improve the overall condition of the |

| |family as a whole. |

| | |

| |The education level in the villages is low. If a physically challenged person who is well-educated lives in the community, it |

| |benefits by his / her knowledge in several ways. He / she can help in educating the children in the village. He / she can also |

| |help in communicating with government agencies to obtain grants / approvals for various schemes benefiting the village |

| |community. He / she can help and teach the village folk about modern advancements in banking. Computer education, electronic |

| |communications |

| |Since this is a project for sponsorship of physically challenged students, we can only sustain it through sponsorship or |

| |donations. We would also sustain it through our income generating units like Gas Agency, Vocational Training Centre, Mobile |

| |Canteen or from sale of agricultural produce from Swapna Nagari, Sindhudurg. |

| |Since we have a commitment to these students, this project would be sustained at all costs. If we do not get sponsorship we may |

| |delay certain capital expenditure on Swapna Nagari project. These children would not drop out of school / college for want of |
