6. Copy of Public Liability Insurance (schedule from Insurance Company)
7. Copy of Food Handler’s Permit(s) (where applicable)
8. Copy of Public Health Food Establishment Certificate (where applicable)
9. Copy of Public Health Swimming Pool Permit (where applicable)
10. Copy of Fire Certificate
11. Approval from Local Planning Authority (e.g. KSAMC and St. James Municipal Corporation)
12. Written approval from the Jamaica National Heritage Trust as a Heritage site (where applicable)
13. Proof of Ownership (eg copy of Land Title) or Certified Lease/Rental Agreement
14. Copy of Surveyor’s ID of property
15. Written permit from NEPA
16. Written approval from NEPA (exemption from Wild Life Protection Act) (where applicable)
17. Permission from River Rafting Authority to Operate on Gazetted River (where applicable)
18. Printed brochures which lists activities offered at the attraction
19. Proper Security Arrangements (i.e. Certified Personnel/copy of Security Contract)
20. Copy of Private Security Regulation Authority (PSRA) Certification for Security Company/Personnel Contracted
21. Security Approval from Resort Areas Division of the Jamaica Constabulary Force
22. List of employees (with job titles)
23. Copy of TPDCo Certification of Tour Guides
24. Copy of Work Permit(s) (where applicable)
25. Copy of Beach Licence from NEPA (where applicable)
26. Letter or Certificate from Government Veterinary (where applicable)
27. Fitness Certificate for motorized transportation used for the touring of property (where applicable)
28. Management plans regarding maintenance , disaster preparedness and safety measures in place and to include, health, safety,& Environmental rules and regulations; training of staff

3. DOCUMENT FOR PERSONNEL INVOLVED IN WATER SPORTS ACTIVITIES (certificates/licences)
- COPY OF FIRST AID AND CPR CERTIFICATE FROM ROYAL LIFE SAVING SOCIETY
- DIVING CERTIFICATE
- LIFEGUARD LICENCE FROM NEPA
- COXSWAIN LICENCE FROM MARITIME AUTORITY OF JAMAICA
- COPY OF SWIFT WATER CERTIFICATION FOR ALL RIVER GUIDES FROM THE ROYAL LIFE SAVING SOCIETY (where applicable)
- FIRST AID KIT
- LIFE SAVING DEVICE – JACKET PER PASSNGER; LIFE RING, SPINAL BOARD
ADDITIONAL CERTIFICATES MAY BE REQUIRED BASED ON THE ACTIVITIES BEING OFFERED
ALL DOCUMENTS MUST HAVE AT LEAST FOUR (4) CLEAR WEEKS BEFORE EXPIRATION AT THE TIME OF APPLICATION
ALSO, ALL GOVERNMENT STATUTORY REQUIREMENTS MUST BE MET AND ADHERED TO.
