The Seller is responsible for obtaining or generating the below disclosures and forms and providing them to the borrower with the initial disclosure package. Copies of the signed documents must be provided to PHL.

FHA ? ALL LOANS:

Initial 1003 URLA, signed and dated by the Loan Officer (1003). Initial HUD 92900A ? Page 1 signed by the LO and page 2 signed by the borrower(s). Initial Loan Estimate (LE) dated within 3 business days of the Initial 1003. Intent to Proceed, acknowledged by customer. FHA Case Number, with Planet Home Lending as the Sponsor. FHA/VA Notice to Applicants. Informed Consumer Choice Disclosure Notice. State-specific disclosures, if any. All other required disclosures per TRID, HMDA and Seller's license requirements.

FHA - Only When Applicable:

FHA ARM Disclosure (ARM Only) Proof of primary residence ? Utility bill or pay stub. (Streamline Only. Can remove income information.) FHA Amendatory / Escape Clause, signed by buyer and seller (Purchase Only) Important Notice to Homebuyers (HUD 92900-B) (Purchase Only) For Your Protection, Get a Home Inspection (Purchase Only) Protect Your Family From Lead In Your Home (Only if the property was built before 1978) Borrower's Acknowledgement ? FHA New Construction (New Construction Only) HUD 92541 ? Builder's Certification of Plans, Specifications and Site (New Construction Only).

FHA Final Submission Disclosure Checklist

FHA ? ALL LOANS: Evidence that the UFMIP (Up-Front Mortgage Insurance Premium) has been paid. Final 1003 URLA, signed and dated by the Borrower(s) and the Loan Officer (1003) Final HUD Addendum, completed per PHL instructions and signed by the Borrower(s)s on Pages 2 & 4. FHA/VA Notice to Applicants. Final Closing Package (Copies of Note, DOT, All LE's and CD's, NORTC, verifications, etc) State-specific disclosures, if any. All other required disclosures per TRID, HMDA and Seller's license requirements.

FHA - Only When Applicable: HUD 92544 ? Warranty of Completion of Construction (New Construction Only).
